乳腺癌外周血微转移与微血管密度相关性研究及临床意义 被引量:3

Correlation between Peripheral Blood Micrometastasis and Microvessel Density and Its Clinical Significance in Breast Cancer

摘要 目的研究乳腺癌外周血微转移与相应肿瘤组织微血管生成的关系,并探讨其临床意义。方法采用逆转录聚合酶链反应(RT-PCR)方法,对50例乳腺癌(乳癌组)及30例乳腺良性肿瘤患者(对照组)外周血中乳腺上皮黏蛋白(small breast epithelial mucin,SBEM)表达情况进行检测;采用免疫组织化学法,用CD105标记肿瘤新生血管,计数乳腺癌和乳腺良性肿瘤组织微血管密度(MVD);分析其相关性及与临床病理特征之间的关系。结果乳癌组SBEM的阳性表达率为36%(18/50),高于对照组的0.0%(0/30)(P<0.05);乳癌组MVD值为24.56±4.12,高于对照组的8.61±2.87(P<0.05);乳癌组SBEM表达与MVD计数呈正相关(r=0.712,P<0.05),与TNM分期、淋巴结转移密切相关(P<0.05);乳癌组MVD计数与肿瘤大小、淋巴结转移、TNM分期有关(P<0.05)。结论乳腺癌外周血微转移与相应癌组织微血管密度密切相关,MVD较高者更易发生外周血微转移,并与肿瘤的发生、发展和转移有关。 Objective To investigate the correlation between peripheral blood micrometastasis and microvascular angiogenesis and its clinical significance in breast cancer.Methods The expres-sion of small breast epithelial mucin(SBEM)in peripheral blood was detected by RT-PCR in 50 patients with breast cancer (breast cancer group)and 30 patients with benign breast tumors (con-trol group).Furthermore,microvessel density (MVD)was counted by labeling new vessels with CD105 antibody.The relationships of MVD to SBEM and clinicopathologic features were ana-lyzed.Results Compared with control group,the positive SBEM expression rate and MVD value significantly increased in control group (36.0% vs 0.0% and 24.56±4.12 vs 8.61±2.87,respec-tively;P〈0.05).The expression of SBEM was positively correlated with MVD in patients with breast cancer(r=0.712,P〈0.05).In addition,SBEM expression was related to lymph node me-tastasis and TNM staging,and MVD count was related to tumor size,lymph node metastasis and TNM staging(P〈0.05).Conclusion The micrometastasis in peripheral blood is closely correla-ted with microvascular angiogenesis in breast cancer.Patients with higher MVD were more likely to have micrometastasis in peripheral blood.The MVD is correlated with the occurrence,develop-ment and metastasis of breast cancer.
