股骨粗隆间骨折DHS与PFNA内固定的微创治疗方法 被引量:9

摘要 目的探讨股骨粗隆间骨折采用闭合复位动力髋螺钉(DHS)和股骨近端防旋髓内钉(PFNA)内固定的微创手术方法。方法回顾性分析自2006-07-2010-12采用闭合复位DHS或PFNA内固定治疗的97例股骨粗隆间骨折,按骨折Evans分型选择内固定方式,其中DHS组40例,PFNA组57例。结果 94例获得随访10~24个月,平均18个月。DHS组骨折愈合时间12~30周,平均20周;PFNA组骨折愈合时间10~18周,平均15周。末次随访时采用d'Aubigne-Postel评分标准评定疗效:DHS组优20例,良9例,可6例,差3例,优良率76.3%;PFNA组优34例,良17例,可3例,差2例,优良率91.1%。结论闭合复位股骨粗隆间骨折后,稳定骨折采用DHS内固定、不稳定骨折采用PFNA内固定效果最好。
