阴式手术对卵巢囊肿患者炎症反应和卵巢功能的影响

Effect of vaginal surgery on inflammation and ovarian function in patients with ovarian cysts

目的探讨阴式手术对卵巢囊肿患者炎症反应和卵巢功能的影响。方法将102例卵巢囊肿患者随机分为两组，对照组51例采用腹腔镜手术治疗，观察组51例采用阴式手术治疗。观察两组手术一般情况（手术时间、术中出血量、术后肠道功能恢复时间、术后住院时间）、卵巢功能和C-反应蛋白（CRP）、降钙素原（PET）的变化。结果观察组在手术时间、术后肠道功能恢复时间、术后住院时间方面均少于对照组（P〈0．05）。术后1个月两组患者的促卵泡素、黄体生成素均明显升高，但对照组较观察组升高更明显（P〈0．05）。两组患者的雌激素、卵巢窦状卵泡数均明显降低，但对照组较观察组降低更明显（P〈0．05）。术后48h两组患者血清CRP和PCT均有明显升高，且对照组较观察组升高更明显（P〈0．05）。结论阴式手术治疗卵巢囊肿，较小影响机体的炎症反应和卵巢功能。

Objective To investigate the effect of vaginal surgery on inflammation and ovarian function in patients with ovarian cysts. Methods One hundred and two patients with ovarian cysts were randomly divided into two groups, the control group （ 51 cases） and the observation group （ 51 cases ）. Patients in the control group were operated through laparoscopy, while the patients in the observation group were operated through vaginal surgery. The general conditions （operative time, blood loss, postoperative intestinal function recovery time, postoperative hospital stay）, ovarian function and C-reactive protein （CRP）, procalcitonin（PCT） changes were observed in the two groups. Results In the observation group, the operative time, postoperative intestinal function recovery time, postoperative hospital stay were lower than those in the control group （P 〈 0. 05）. After 1 month, the levels of FSH, LH in the two groups were significantly increased, but those in the control group were increased more significantly than those in the observation group （P 〈 0.05 ）. The levels of E2, F0 in the two groups were significantly reduced, but those in the control group were decreased more significantly than the observation group （P 〈 0. 05）. Forty-eight hours after operation, the serum CRP and PCT levels of the two groups were significantly increased, and those in the control group were increased more significantly than those in the observation group （P 〈 0. 05）. Conclusions Vaginal surgery in the treatment of ovarian cysts has less affection on the inflammatory response and ovarian function.
