结构变动度在住院费用构成变动中的应用

The Application of Composition Change Degree in the Change of Hospitalization Expense Composition

目的探讨降低医疗费用的对策。方法运用结构变动度和环比增长速度指标,分析2008年-2013年费用构成变动的情况。结果费用构成变动比较合理,其他费和检查费是引起费用结构变动上升的主要费用项目,二者累积贡献率高达42.44%,护理费结构变动增幅较慢,仅为4.10%,说明在体现医生技术服务价值的同时,也应适当提高护理人员的技术服务价值,药品费、诊疗费、手术费是引起住院费用结构变动下降的主要费用项目,三者累积贡献率高达45.92%,说明了我院优化了收费结构,强化了管理意识。结论优化医疗收入结构,有重点的控制主要因素,以较低的成本及合理收费,最大限度的满足患者医疗需求,实现医疗费用的理性回归。

Objective To investigate measures to reduce medical treatment expense.Methods To analyze the change of expense composition in the past 6 years with the application of composition change degree and index of ring growth rate.Results The change of expense composition was reasonable,other fees and inspection fees were major cause of the changes in the expense structure,which contributed 42.44％ to the growth of expense.The growing rate of changes in the nursing costs structure was low,only 4.10％,which indicated that at the same time to embody the value of medical service of the doctors,we should increase the value of nursing services.Drug costs,medical costs,operation costs were three major parts decreased the hospitalization costs,which contributed the rate up to 45.92％,and which indicated that our hospital had an optimization on the expense structure,and strengthened the awareness of management.Conclusions To optimize the income structure of medical treatment,control the main variables intensively,to satisfy the medical treatment needs of patients to the maximum extent with lower costs and reasonable fees,thus to achieve rational regression of medical expenses.
