负压封闭引流术在小面积皮肤软组织缺损中的应用

CLINICAL APPLICATION OF VSD FOR SMALL AREAS OF SKIN AND SOFT TISSUE DEFECT

目的探讨负压封闭引流术(VSD)在小面积皮肤软组织缺损中的临床应用效果。方法对近两年我科收治的45例小面积皮肤软组织缺损病人进行回顾性分析,其中13例病人采用常规清创换药+游离植皮(A组),32例采用清创VSD覆盖+游离植皮(B组)。比较两组治疗时间、治疗过程中的疼痛程度和病人对愈后瘢痕的接受度。结果 B组与A组相比,平均治疗时间短,治疗过程中疼痛轻,病人对愈后的瘢痕更容易接受,差异具有统计学意义(t=3.016、3.253,χ2=4.102,P<0.05)。结论在小面积皮肤软组织缺损的治疗中,采用清创VSD覆盖+游离植皮的疗效显著优于采用常规清创换药+游离植皮。

Objective To explore the clinical application of vacuum sealing drainage （VSD） technique in the treatment of small areas of skin and soft tissue defect. Methods A retrospective review was done in 45 patients with small-area skin and tis- sue defect treated in the last two years in our department, of whom, 13 were treated with debridement and change dressing plus free skin grafting （group A）, and 32 patients received VSD after debridement plus free skin grafting （group B）. The time of thera- py, pain during the treatment, and the acceptance of the scar after treatment were compared between the two groups. Results The average time of treatment was shorter, pain during treatment was milder, and the scar appearance was more acceptable in group B than group A, the differences between the two groups were statistically significant （t =3.016,3.253;X2=4.102;P〈0.05）. Conclusion Combined VSD with free skin grafting in the treatment of small-area skin and tissue defect can get a much better out come than using traditional debridement dressing plus free skin grafting.
