某市高技能人才高血压、糖尿病医疗费用的变化趋势

Trends in Medical Expenses on Hypertension and Diabetes of Highly Skilled Personnel in a City

[目的]了解苏州市高技能人才高血压和糖尿病医疗费用支出的变化趋势,为该人群相关慢性疾病的防治及医疗经费增长的控制提供依据。[方法]从苏州医疗保险信息数据库中选取苏州城区机关企事业单位参保的高级技工47 129人为研究对象,收集该人群2001—2012年高血压、糖尿病门诊与住院的医疗数据,并进行统计分析。[结果]12年间,高血压现患3 539人(占7.51%)、糖尿病832人(占1.77%)。高血压、糖尿病患者门诊及住院总医疗费用分别为1.769 87亿元、1.291 84亿元,分别占该人群总医疗费用的14.27%和10.42%。糖尿病患者人均门诊及住院医疗费用高于高血压患者(P<0.01),男性人均费用高于女性(P<0.01)。高血压患者中男性人均门诊医疗费用高于女性(P<0.01),但不同性别患者的住院费用差异无统计学意义(P>0.05)。高血压、糖尿病医疗总费用的年均增长率分别为21.95%和19.28%。两种疾病年医疗总费用及其增长速度总体呈逐年上升趋势。[结论]高血压、糖尿病的医疗费用在苏州市高技能人才医疗总经费中所占比例较大,且呈逐年上升趋势。

[ Objective ] To observe the trends in medical expenses on hypertension and diabetes of highly skilled personnel （lISP） in Suzhou city, and to provide basis for nutrition-related non-infectious chronic diseases （NCDs） intervention and relevant medical expenses growth control towards HSP. [ Methods ] A total of 47129 HSP having senior technician certificate and entitled with social medical insurance were recruited to the current study. The medical data on outpatient or inpatient of patients diagnosed with hypertension and diabetes were retrieved from the Database of Suzhou Medical Insurance Information within the period of 2001 to 2012. Statistical analysis were applied to the obtained data. [ Results ] From 2001 to 2012, 3 539 hypertension patients （7.51%） and 832 diabetes patients （1.77%） were identified in the database. The medical expenses （outpatient and inpatient） on hypertension and diabetes reached RMB 176.987 million yuan （14.27%） and RMB 129.184 million yuan （10.42%） respectively. The per capita medical expenses （outpatient and inpatient） on diabetes was higher than that on hypertension （P〈 0.01）. The per capita medical expenses （outpatient and inpatient） on diabetes in the male HSP was higher than that in the female （P 〈 0.01）. For patients with hypertension, per capita outpatient medical expenses in the males was higher than that in the females （P〈 0.01）, and the gender difference in inpatient medical expenses was not statistically significant （P 〉 0.05）. The average annual growth rates of medical expenses （outpatient and inpatient） on hypertension and diabetes were 21.95% and 19.28% respectively. In general, the annual medical expenses （outpatient and inpatient） and related growth rates （fixed base ratio） on hypertension and diabetes displayed a yearly growing tendency, except for a decline in 2012. [ Conclusion ] Medical expenses on hypertension and diabetes consume a large proportion of the total medical expenses towards HSP in Suzhou city and show a growing tendency.
