某“三甲”医院静脉药物配置中心不合理用药医嘱分析

Analysis of Irrational Medication Orders in Pharmacy Intravenous Admixture Services of a“Three-A”Level Hospital

目的:了解某"三甲"医院静脉药物配置中心不合理用药医嘱情况,为提高医院合理用药水平提供参考。方法:收集该院静脉药物配置中心2012年的不合理用药医嘱并精心整理,主要以药品说明书、《中国药典·临床用药须知》(2010年版)、《新编药物学》(17版)和相关专业书籍等为依据,对不合理用药医嘱进行统计分析。结果:该院不合理用药医嘱主要表现在溶媒选择、药物配伍、药物剂量、溶媒用量、用药方案和用药次序等方面不合理。结论:临床药师应进一步提高审方能力,积极和及时与临床医师沟通,纠正不合理用药医嘱,以加强对药物配伍的监管,减少药品不良事件的发生,有效保障临床用药安全。

OBJECTIVE：To investigate the irrational medication orders in the intravenous admixture services of a“Three-A”level hospital so as to provide references for improving rational drug use level in hospital. METHODS：The irrational medication or-ders collected in this hospital during 2012 were sorted carefully and analyzed in accordance with package inserts,Pharmacopoeia of the People’s Republic of China- Clinical dispensing information（2010 version）,New Pharmacology（17th version） and other related technical books. RESULTS：The irrational medication orders were irrational in solvent selection,compatibility of drugs,dosage, amount of solvents,dosage schedule and sequence of medication etc. CONCLUSIONS：Pharmacists should improve their ability in checking prescription,positively and timely communicate with the physicians to correct the irrational mediation orders and intensify monitoring on compatibility of drugs so as to reduce the incidence of adverse drug event and ensure clinical medication safety.
