阿加曲班与奥扎格雷对急性缺血性脑卒中的临床疗效比较

Comparison of the therapeutic effects between argatroban and ozagrel on acute ischemic stroke

目的：探讨阿加曲班对急性缺血性脑卒中的临床疗效及最佳治疗时机。方法：将收治的73例急性缺血性脑卒中患者按数字随机表法分为治疗组38例和对照组35例；对照组给予奥扎格雷，治疗组静脉泵入阿加曲班。再将治疗组患者按照24h内不同给药时段分为治疗1组（发病6～12h使用阿加曲班）和治疗2组（发病13-24h使用阿加曲班）。对比治疗前后患者NIHSS评分、血小板计数和活化部分凝血酶原时间；记录患者不良反应发生情况。结果：治疗组与对照组血小板计数、APTT、差异无统计学意义（P〉0．05），治疗组NIHSS评分显著低于对照组（P〈0．05）。治疗1组与治疗2组血小板计数、APTT、NIHSS评分相比差异无统计学意义（P〉0．05）。结论：阿加曲班可以显著改善急性缺血性脑卒中患者的神经功能；24h内不同时段给予阿加曲班对疗效无明显影响。

OBJECTIVE To compare the therapeutic effects between argatroban and ozagrel on acute cerebral ischemie stroke and explore their best treatment time. METHODS 73 cases of patients with acute ischemic stroke in our hospital were random ly divided into treatment group of 38 cases and control group of 35 cases, the control group was given ozagrel, the treatment group was given with intravenous argatroban The 38 patients in the treatment group according to the different administration time were divided into treatment group I （the use of argatroban at the incidence of 6-12 h） and treatment group 2 （the use of argatroban at the incidence of 13-24 h）. Patients＂ NIHSS score, platelet count and activated partial thromboplastin time were compared before and after treatment; and the occurrence of adverse reactions was recorded. RESUUTS The platelet count, APTT of treatment group and control group had no significant difference（P〉0. 05）, the NIHSS of the treatment group was significantly lower than that of control group（P〈0. 05）. The treatment group 1 and treatment group 2 platelet count, APTT, NIHSS score had no significant difference（P〉0. 05）. CONCLUSION Argatroban can significantly improve the nerve function of patients with acute ischemic stroke, and within 24 h given argatroban had no obvious different effects.
