广州市荔湾区网格化社区慢性病管理模式研究

Grid management model for managing residential community chronic diseases at Liwan District, Guangzhou

目的 探索网格化管理模式对社区慢性病管理的意义.方法 本中心自2011年开始实施社区卫生服务网格化管理,比较网格化管理实施前、后居民健康档案的建档指标、慢性病管理指标和社区居民的满意度.结果 实行网格化管理后,居民健康档案的建档指标以及慢性病管理指标明显上升,社区居民的满意率明显提高.结论 通过网格化管理可以有效提高社区慢性病管理工作的质量,提高社区居民的满意度.

Objective To exlpore the significance of grid management model for managing residential community chronic diseases. Methods Grid management has been put into practice at our center since 2011. The data of the residents＇ health archive, the effect of chronic disease management and the residents＇ satisfactory degree were compared between before and after the management was carried out. Results Since the grid management model was adopted, the number of the residents＇ health archive has increased a lot, the effect ofchronic disease management has greatly improved, and residents＇ satisfactory degree has grown much higher. Conclusions Grid management can significantly improve the performance of chronic disease management and the residents＇ satisfaction.
