糖尿病社区管理的效果分析

Analysis of the Effect of Community Management of Diabetes

目的 了解南宁市糖尿病社区教育管理的效果.方法 将2008年1～12月南宁市江南区的2个社区中符合入选标准的55例无亲缘关系的2型糖尿病患者作为研究对象,随机选取1个社区作为干预社区,另外一个为对照社区,监测2个社区中糖尿病患者管理前后的各项生化等指标,评价南宁市糖尿病患者社区教育管理的效果.结果 对社区糖尿病患者进行规范管理后,其收缩压、舒张压、月费用、体质量指数、糖化血红蛋白、空腹血糖、餐后2h血糖及尿蛋白的下降幅度显著大于对照社区（P＜0.05）.干预社区患者糖尿病视网膜病变、周围神经病变的发生率显著低于对照社区（P＜0.05）,冠状动脉硬化性心脏病及脑卒中的发生率比较无统计学意义（P＞0.05）.结论 通过对社区糖尿病患者进行教育管理,可以有效控制糖尿病的发展,减少医疗费用,提高患者的生活质量.

Objective To evaluate the effects of community manag t of type 2 diabetes mellitus （T2DM） in Nanning. Methods The effects of community management of T2DM in Jiangnan district in Nanning were observed in all subjects selected from two communities during Jan. and Dec. 2008. The 55 cases selected were unrelated type 2 diabetic patients meeting the inclusion criteria,and one community was set as the intervention group while the other as the control group randomly. The effect of education manage- ment on the diabetic patients was then assessed. Results After standard management of the diabetic patients, SBP, DBP, BMI, HbA1 c, FPG, PPG-2h and urinary protein decline of the intervention group were significantly greater than the control group（ P 〈 0.05 ）. The incidences of diabetic retinopathy and diabetic peripheral neuropathy of the intervention group were significantly lower than the control group（ P 〈 0. 05 ）. The incidences of coronary arteriosclerotic heart disease and stroke between the two groups were not statisti- cally significantly different （ P 〉 0.05 ）. Conclusion It can effectively control the development of diabetes, reduce medical costs and improve the patient＇s quality of life through community education and management of patients with diabetes.
