摘要
目的探讨腮腺淋巴上皮瘤样癌(lymphoepitheloma-like carcinoma,LELC)的MRI表现,旨在提高对该病的认识。方法回顾性分析经病理证实的8例腮腺LELC的MRI表现。结果 8例腮腺LELC均为左侧实性肿块,单发3例,多发5例。深、浅叶均有者4例,浅叶者3例,跨深浅叶者1例。平扫T1WI呈等或稍高信号4例,等信号4例;T2WI呈高信号6例,稍高信号2例;增强后均呈现早期明显强化。完整及不完整包膜并存者4例,不完整分隔者2例,完整包膜者2例,包膜均呈长T1、等T2信号,增强后逐渐延迟强化。边界大部分清晰局部模糊者2例,清晰3例,模糊不清3例。有邻近皮下脂肪、皮肤侵犯6例,无2例。邻近区域有淋巴结转移者6例,无2例。结论 MRI能反映腮腺LELC的影像特点,具有一定特征性。
Objective To investigate the MRI features of lymphoepithelioma-like carcinoma ( LELC) of the parotid gland in order to improve its recognition .Methods The MRI features of 8 cases with LELC of parotid gland proved by pathology were analyzed .Results All cases were solid masses on the left side of parotid gland , single in 3 cases, multiple in 5 cases.Four cases were situated in superficial lobe and deep lobe of the parotid gland , and 3 ca-ses were situated in superficial lobe , and 1 case was situated between superficial lobe and deep lobe .T1-weighted im-aging showed 4 cases with isointense or slight hyperintense signals , 4 cases with isointense; T2-weighted imaging demonstrated hyperintense in 6 cases, slight hyperintense in 2 cases.All lesions showed obvious enhancement in the early phase of MRI .There were 4 cases with complete or incomplete capsule , 2 cases with the incomplete partition , 2 cases with complete capsule .The capsules demonstrated hypointense signal on T 1-weighted imaging and isointense signal on T2-weighted imaging , which demonstrated delayed enhancement .In 8 cases MRI displayed almost clear borders in 2 cases, clear borders in 3 cases, unclear borders in 2 cases.MRI displayed the subcutaneous fat and skin were involved in 6 cases, and not involved in 2 cases.Regional lymph node metastases were found in 6 cases, were not found in 2 cases.Conclusion LELC of parotid gland exhibits some imaging features on MRI , which can be the clue for the diagnosis .
