非酒精性脂肪肝患者血清免疫球蛋白与肝纤维化指标的相关性分析

Correlation between Serum Immunoglobulin and Indexes of Liver Fibrosis in Patients with Non-Alcoholic Fatty Liver Disease

目的探讨非酒精性脂肪肝(NAFLD)患者的血清免疫球蛋白(Ig)与肝纤维化指标的相关性。方法检测168例NAFLD患者和200例健康者(对照组)的血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、γ-谷氨酰转移酶(GGT)、白蛋白(ALB)、IgA、IgG、IgM、血清Ⅲ型前胶原(PⅢNP)、Ⅳ型胶原(CⅣ)、层粘连蛋白(LN)、透明质酸(HA),并分析其相关性。结果 NAFLD组患者血清ALT、AST、GGT、IgA、IgG、IgM、总Ig、PⅢNP、CⅣ和HA水平明显高于对照组(P<0.05);NAFLD组患者血清IgA、IgG、IgM与PⅢNP、CⅣ、LN、HA水平呈正相关关系(P<0.05);血清IgA、IgM、IgG在轻度、中度和重度NAFLD患者中呈逐步递增趋势(P<0.05)。结论 NAFLD患者血清Ig水平与肝纤维化指标呈正相关关系;检测NAFLD患者血清Ig水平,对判断肝纤维化进程有重要作用。

Objective To investigate the correlation between serum immunoglobulin （ Ig） and indexes of liver fibrosis in patients with non-alcoholic fatty liver disease （ NAFLD ） .Methods The serum alanine aminotransferase （ALT）,aspartate aminotransferase （AST）,γ-glutamyl transferase I （GGT）,albumin（ALB）,IgA,IgG,IgM,serum procollagen type Ⅲ（PⅢNP）,Ⅳcollagen（CⅣ）,laminin（LN） and hyaluronic acid（HA） were detected in 168 cases of NAFLD and the 200 healthy controls.The correlation was analyzed as well .Results The levels of ALT,AST,GGT, IgA,IgG,IgM,total Ig,PⅢNP CⅣ,LN and HA in NAFLD group were significantly higher than those in the control group（P〈0.05）.The levels of IgA,IgG,IgM positively correlated with PⅢNP,CⅣ,LN,HA levels（P〈0.05）.The serum IgA,IgM,IgG in mild,moderate and severe NAFLD patients had an increasing trend （P〈0.05）.Conclusion The level of serum Ig in patients with NAFLD positively correlates with indexes of liver fibrosis .The detection of serum Ig in patients with NAFLD plays an important role in diagnosing the process of liver fibrosis .
