超声二维斑点技术评价缩窄性心包炎患者左室运动功能的价值 被引量:1

Ultrasonic Two-Dimensional Speckle Tracking Imaging Applied to Quantitative Evaluation on Left Ventricular Movement Function in Patients with Constrictive Pericarditis

摘要 目的探讨超声二维斑点追踪技术(2D-STI)定量评价缩窄性心包炎(CP)患者左室纵向运动功能的价值。方法 CP患者40例(CP组),健康志愿者40例(对照组),应用2D-STI技术测量两组观察对象心尖3个长轴(四腔心切面、二腔心切面及左心室长轴切面)总体纵向应变值及左心室各个节段纵向应变值。结果 CP组患者心尖3个长轴(四腔心切面、二腔心切面及左心室长轴切面)总体纵向应变值明显低于对照组(P<0.05);CP组患者有心包覆盖区域(侧壁、前壁、后室间隔基底段、后室间隔心尖段、下壁中间段、下壁心尖段、后壁中间段、后壁心尖段、前间隔心尖段及前间隔中间段)纵向应变值明显低于对照组(P<0.05),无心包覆盖区域(后室间隔中间段、下壁基底段及前间隔基底段)纵向应变值与对照组比较,差异无统计学意义(P>0.05)。结论CP患者心脏总体纵向运动降低,病变早期有心包覆盖区域心肌存在损害;2D-STI技术能定量评估CP患者心肌功能,反映心肌损伤情况。 Objective To investigate the clinical value of two-dimensional speckle tracking imaging (2D-STI) in evaluation on left ventricular longitudinal movement function in patients with constrictive pericarditis (CP).Methods Forty patients with CP(CP group) and 40 healthy volunteers(control group) were measured with 2D-STI for left ventricle overall longitudinal strain and per section longitudinal strain in three sections ( four-chamber view ,two-chamber view and left ventricular long-axis view ) .Results The three longitudinal strains of CP group were significantly lower than those of control group ( P〈0 .05 ) .The left ventricle section longitudinal strains with pericardium covered area of CP group were significantly lower than those of control group ( P〈0 .05 ) .Those without pericardium covered area showed no significant difference between CP group and control group ( P〉0 .05 ) .Conclusion Left ventricle overall longitudinal strain reduces in CP patients .Myocardial damage exists in pericardium covered area of early stage CP .2 D-STI can be applied to quantitatively evaluating myocardial function as well as reflecting characteristic of myocardial damage in CP patients .
