基层医院开展胎儿超声心动图产前筛查的临床价值

摘要 目的探讨基层医院开展胎儿超声心动图产前筛查的临床价值。方法选择我院5 000例中期孕妇采用彩色多普勒超声显像仪进行产前大畸形筛查和胎儿心脏针对性检查,筛查出胎儿心脏畸形的超声图像表现与发生心内、心外和染色体病变的关系,比较超声诊断与病理诊断的符合率。结果 5 000例胎儿心动图产前筛查中,检查出41例心脏畸形胎儿,检出率为8.20‰;病理活检检出胎儿心脏畸形43例,检出率8.60‰;两者诊断符合率95.35%。结论胎儿超声心动图对胎儿心脏畸形的诊断符合率高,与病理诊断结果无显著性差异,是一项安全、有效的产前筛查技术,值得基层医院推广应用。 Objective To explore the clinical value of fetal echocardiography in prenatal screening in local hospital. Methods 5 000 cases of second-trimester pregnancy were examined by color Doppler ultrasonography in our hospital for fetal heart malformation and prenatal malformations. The primary outcome was a correlative analysis between the echocardiography features of fetal cardiac malformation and endocardiac, exocardial and chromosomal diseases. The secondary outcome was a comparison between the compliance rate of ultrasound diagnosis and pathologic diagnosis. Results In 5 000 cases of fetal echocardiography prenatal screening, 41 cases were diagnosed fetal cardiac malformations, with the detection rate of 8.2%. The pathologic detection found 43 cases of fetal cardiac malformation, with the detection rate of 8.6%. The diagnostic compliance rate was 95.35%. Conclusions The diagnostic compliance rate of fetal echocardiography for fetal cardiac malformations is high, without significant difference in contrast to pathological diagnostic results, which is a safe and effective prenatal screening technique and deserves application in local hospitals.
