奥拉西坦注射液联合甘露醇治疗脑出血后脑水肿疗效分析

Efficacy analysis of oxiracetam injection combined with mannitol in treatment of brain edema after intracranial hemorrhage

目的探讨奥拉西坦注射液联合甘露醇治疗脑出血后脑水肿疗效。方法将80例颅内出血患者随机分成观察组和对照组各40例。对照组采用20%甘露醇等常规治疗,观察组在20%甘露醇的基础上加用奥拉西坦注射液4.0加入0.9%氯化钠250 ml静脉静滴,每天1次,连续7 d。比较2组患者治疗前后血肿大小及水肿体积;同时对治疗前后神经功能缺损程度(NIHSS评分)和日常生活活动能力(Bathel指数评分)进行评分。结果 2组自身在治疗前后比较,血肿大小、水肿体积明显缩小(P<0.05),CPK值明显降低(P<0.05),观察组较对照组改善更明显,(P<0.05);2组患者治疗9 d,NIHSS评分和日常活动能力评分各自均明显优于治疗前(P<0.05),而观察组较对照组改善更明显(P<0.05);观察组显效率为75%,对照组显效率为57%,观察组优于对照组(P<0.05)。结论奥拉西坦注射液联合甘露醇治疗脑出血后脑水肿效果优于单一用甘露醇的常规治疗。

Objective To investigate the efficacy of oxiracetam injection combined with mannitol in treatment of brain ede- ma after intracranial hemorrhage. Methods 80 cases of intracranial hemorrhage patients were randomly divided into an obser- vation group and a control group. The control group was given routine treatment with 20% mannitol injection, while the obser- vation group,in contrast, was coupled with oxiracetam injection. The patients of the two groups were compared before and after the treatment in terms of the hematoma size and edema volume. In the meantime, the patients＇neurological deficit （by NIHSS score } and activities of daily living （ by Bathel index score） before and after the treatment were graded and evaluated. Results After the treatment, the hematoma size and edema volume of the two groups＂ patients significantly reduced （ P 〈 O. 05 ） , and CPK value was significantly lower{ P 〈 O. 05 ）. In comparison, the observation group improved more apparently （ P 〈 0.05 ）. The patients of the two groups remarkably ameliorated after the treatment in terms of NIHSS score and activities of daily living score （ P 〈 0.05 ） , but the observation group improved better（ P 〈 0.05 }. The remarkable efficiency rate was 75 % for the observation group and 57% for the control group. The observation group had an advantage over the control group（ P 〈 0. 05 ）. Conclusion In comparison with the routine treatment with mannitol, oxiracetam injection combined with mannitol can more effectively ab- sorb brain edema.reduee intracranial pressure, thus improve patients＂ neurological condition and their quality of life.
