浙江省嘉善县农村社区中老年男性下尿路症状的流行病学调查

The prevalence of lower urinary tract symptoms in middle-aged and elder males in a rural community of Jiashan County, Zhejiang Province

摘要 目的:调查浙江省嘉善县农村社区中老年男性下尿路症状(LUTS)的流行病学情况。方法2013年4月至10月在浙江省嘉兴市嘉善县魏塘乡村选取598例50岁以上的中老年男性,通过国际前列腺症状评分(IPSS)和生活质量(QOL)评分等方法调查该人群中LUTS的患病率、对生活质量的影响以及药物治疗的情况。结果共有21.7%的被调查者具有中、重度LUTS(IPSS≥8)症状,在50岁~、60岁~、70岁~和>80岁年龄组中,中、重度LUTS患病率分别为13.9%,20.0%,40.0%和40.8%。中、重度LUTS患病率随年龄增长和个人收入增加呈上升趋势;随着LUTS严重程度的增加,他们的QOL评分也显著上升。寻求就诊药物治疗的老年中、重度LUTS男性的比率为27.7%,并随着QOL评分和个人收入状况而增加。结论浙江省嘉善县农村社区中老年男性LUTS与国内外报道的患病率接近,并随着年龄增长和个人收入增加呈上升趋势。接受药物治疗的患者不到30%。 Objective To explore the prevalence of lower urinary tract symptoms (LUTS), quality of life (QOL) and intervention-seeking behavior in middle-aged and elder menin rural area of Southern China. Methods Total of 598 men with over 50 years old were enrolled in this study. The interviewer-administered questionnaire including International Prostate Symptom Score (IPSS) and quality of life (QOL) were used to assess the score of symptom severity. Results About 21.7% of the test subjects had moderate-to-severe LUTS (IPSS≥8). In the group of 50 to 59, 60 to 69,70 to79 and ≥80 years old,the incidence rates of moderate and severe LUTS were13.9%, 20.0%, 40.0% and 40.8%,respectively.The severity of LUTS was found to be increased with the growing of age and the increase of individual annual income. The scores of quality of life increased with the grade of LUTS severities. The rate of medicine-seeking respondents with moderate-to-severe LUTS was 27.7% and increased with their QOL scores and individual annual income. Conclusion The prevalence of LUTS of old-aged males in the rural community of Zhejiang Province was close to that in the Chinese urban areas and in Western countries reported in other studies. However, 〉70% of patients with moderate-to-severe LUTS did not seek medical intervention for symptoms.
