前列腺弥漫大B细胞型非霍奇金淋巴瘤2例并文献复习

Primary diffuse large B-cell non-Hodgkin’s lymphoma of the prostate：2 cases report and literature review

目的：探讨原发于前列腺弥漫大B细胞型非霍奇金淋巴瘤的临床特点、诊断及治疗。方法报道2例原发于前列腺弥漫大B细胞型非霍奇金淋巴瘤患者的临床表现及特点、诊断及治疗方案，并结合文献进行讨论。结果原发于前列腺弥漫大B细胞型非霍奇金淋巴瘤临床表现常有血尿、尿频尿急、排尿困难及出血倾向。2例患者行经直肠彩超引导下前列腺穿刺活检术，病理及免疫组织化学结果均提示为前列腺弥漫大B细胞型非霍奇金淋巴瘤。1例患者给予R-CHOP（利妥昔单抗、环磷酰胺、阿霉素、长春新碱、泼尼松）方案化疗，至今仍在随访中；还有1例患者出院后失访。结论原发于前列腺弥漫大B细胞型非霍奇金淋巴瘤临床罕见，前列腺穿刺活检可以明确诊断，化疗为主要治疗手段，预后受多种因素影响。

Objective To explore the clinical characteristics, diagnosis and treatment of primary diffuse large B-cell non-Hodgkin’s lymphoma of the prostate. Methods Two cases of primary diffuse large B-cell non-Hodgkin’s lymphoma of the prostate were studied by analyzing the clinical manifestations and characteristics, diagnosis, treatment and review of the literature. Results The clinical manifestations of primary diffuse large B-cell non-Hodgkin’s lymphoma of the prostate was hematuria, urinary frequency and urgency, dysuria and bleeding tendency. 2 cases were underwent transrectal prostate biopsy, the pathologic and immunohistochemical results showed that were diffuse large B-cell non-Hodgkin’s lymphoma, 1 patient received R-CHOP chemotherapy and still under follow-up, 1 patient failed to follow-up after discharge. Conclusion Primary diffuse large B-cell non-Hodgkin’s lymphoma of the prostate is rare, transrectal prostate biopsy can confirm the diagnosis, chemotherapy is the major treatment, prognosis is influenced by various factors.
