盐酸坦洛新治疗输尿管结石所致肾绞痛的临床观察研究

CLINICAL OBSERVATION AND RESEARCH OF TAMSULOSIN HYDROCHLORIDE TREATMENT ON RENAL COLI CAUSED BY TO URETERAL CALCULUS

目的：探讨α1肾上腺素受体阻断剂盐酸坦洛新缓释片对输尿管结石并肾绞痛的疗效。方法：228例直径0.4-0.7 cm的输尿管结石所致肾绞痛患者,分为2组,每组114例,其中坦洛新,0.4 mg,每日1次口服,4周;硝苯地平,10 mg,每日3次口服,4周。结果：试验组（硝苯地平＋坦洛新组）,治疗期间肾绞痛发作15次,60分钟肾绞痛缓解有效率86.61%,结石排出率84.82%,平均排出时间（7.21±2.58）天;对照组（硝苯地平组）治疗期间,肾绞痛发作52次,60分钟肾绞痛缓解率53.51%,结石排出率51.35%,平均排出时间（12.86±6.46）天。两组治疗期间,肾绞痛发生率及结石排出率比较有显著性差异（P〈0.05）。结论：盐酸坦洛新缓释片治疗输尿管结石能明显提高60分钟肾绞痛缓解率,明显减少治疗期间肾绞痛的发作次数。

Objective： To investigate the effect of α1 acceptor blocker. Tamsulosin hydrochloride sustained-release tablets for treatment of renal colic caused by ureteral stones. Method： 228 patients were divided into two groups randomly, 114 cases for each group, with the ureteral stones of diameter 0.4 - 0.7 cm. The group of tamsulosin hydrochloride sustained-release tablets, 0.4 mg once daily orally, continued for 4 weeks. The group of Nifedipine was given 10 mg three times a day orally, continued for 4 weeks. Both groups are required to drink water more than 2000 ml water daily. Results： In the test group（tamsulosin）, renal colic occurred 15 times during the treatment. The reliefe of renal colic efficiency was 86.61% in 60 minutes and the stone expulsion rate was 84.82%. The average time of expulsion was（7.21±2.58） days; In the control group, the renal colic occurred 52 times during the treatment. The reliefe of renal colic efficiency was 53.51% in 60 minutes and the stone expulsion rate was 51.15%. The average time of expulsion was（12.86±6.46） days. There was a significant difference in the incidence of renal colic and stone expulsion rate between the two groups（P〈0.05）. Conclusion： Tamsulosin hydrochloride sustained-release tablets were effective for the treatment of ureteral stones in improving 60 minutes remission rate of renal colic, reducing the episodes of renal colic and shortening the time of stone expulsion.
