摘要
目的评价三维超声在产前诊断胎儿严重手畸形中的价值。方法对2 612例孕妇进行二维和三维超声检查,以排除胎儿严重手畸形。对产前超声诊断严重手畸形的胎儿进行产后随访,比较二维超声与三维超声对胎儿严重手畸形的显示情况,统计学分析采用χ2检验。结果产前超声检出胎儿严重手畸形101例(手缺如4例、裂手畸形5例、缺指畸形17例、桡骨发育不全或缺如16例、上肢远端及手缺如8例、手腕部姿势异常7例、致死性骨发育不良34例、重叠指1例、多指7例、并指2例),产前超声误诊1例。结论三维超声为胎儿严重手畸形的诊断提供了重要手段,是二维超声的重要补充。
Objective To determine the value of three-dimensional(3D) ultrasonography in prenatal diagnosis of fetal severe hand deformity. Methods A total of 2 612 pregnant women underwent two-dimensional(2D) and 3D ultrasonography to detect fetal severe hand deformity. The findings of 2D and 3D ultrasonography were compared. Chi-Square test was used to analyze diagnosis accuracy of 2D and 3D ultrasonography. Results 101 cases of fetal severe hand deformity were diagnosed with prenatal ultrasonography(4 were absent hand, 5 were cleft hand, 17 were ectrodactyly, 16 were congenital hypoplasia or aplasia of the radius, 8 were distal upper limb and hand absence, 7 were abnormal wrist posture, 34 were lethal skeletal dysplasia, 1 was overlapping hand, 7 were polydactyly, and 2 were syndactyly), and 1 case was misdiagnosed. Conclusion 3D ultrasonography provides an important tool for diagnosing fetal severe hand deformity, and it is an important complementary tool to 2D ultrasonography.
