“益气活血”法治疗慢性阻塞性肺疾病稳定期(气虚血瘀证)的临床研究

Effects of “Reinforcing Qi and Activating Blood” on Stable Chronic Obstructive Pulmonary Disease(Qi Deficiency and Blood Stasis Syndrome)

目的研究"益气活血"法治疗慢性阻塞性肺疾病(COPD)稳定期(气虚血瘀证)患者的临床疗效。方法选择COPD稳定期(气虚血瘀证)患者50例,随机分为对照组和治疗组。对照组给予基础干预,治疗组在基础干预的基础上加用玉屏风胶囊(2粒tid.)和复方丹参片(3片tid.)治疗,共治疗3月,观察12月。观察治疗前后的中医症状积分,评价中医证候疗效,观察细胞免疫指标及血流动力学指标的变化,同时对终点指标进行评价。结果治疗组的总有效率在治疗3月和随访3月时明显优于对照组(P<0.05),在中医单项症状方面,治疗组对气短和自汗的改善优于对照组(P<0.05),治疗组疗后的CD4+水平较疗前升高(P<0.05),其疗后CD8+水平降低(P<0.05)、CD4+/CD8+比值升高(P<0.05),治疗组疗后NO水平升高(P<0.05)、内皮素(ET)水平降低(P<0.05),两组间疗后ET水平比较差异有统计学意义(P<0.05);治疗组患者的年感冒次数、住院次数及住院日均少于对照组(P<0.05)。结论 "益气活血"法能够较好的改善COPD稳定期(气虚血瘀证)患者的中医症状并提高疗效,调节患者的细胞免疫功能和血管的舒缩功能。

Objective To study the clinical effects of＂Reinforcing Qi and Activating Blood＂in the treatment of stable chronic obstructive pulmonary disease（Qi Deficiency and Blood Stasis Syndrome）.Methods This study recruit 50outpatients with stabe chronic obstructive pulmonary disease（Qi Deficiency and Blood Stasis Syndrome）,who were randomly divided into 2groups,control group and treatment of＂Reinforcing Qi and Activating Blood＂group.The patients in control group were given conventional treatment,while the patients in treatment group not only received conventional treatment but also the treatment of Yu-Ping-Feng capsules and Fu-Fang-Dan-Shen tablets for 3months.The duration of follow up was 12months,and chinese medicine（CM）symptoms,cellular immunity indexes and haemodynamics indexes were assessed every 3months.Results The total effective rate in treatment group at 3months treatment and 3months follow-up were higher than that in control group（P＆lt;0.05）.The CM symptoms in treatment group were improved significantly（P＆lt;0.05）.In treatment group,the level of CD4and the ratio of CD4/CD8after therapy were higher than those before（P＆lt;0.05）,and the level of CD8was lower than that before（P＆lt;0.05）.The level of NO was no significant difference（P＆gt;0.05）,while the level of endothelin（ET）was significantly different（P ＆lt;0.05）.The frequency of suffering from cold,the admission due to acute exacerbation and hospital stay in treatment group were all lower than those in control group（P＆lt;0.05）.Conclusion The＂Reinforcing Qi and Activating Blood＂therapy can improve the symptoms and clinical curative effect of TCM in the treatment of stable COPD（Qi Deficiency and Blood Stasis Syndrome）,with the improvement of their cellular immunological function and vasomotor function.
