摘要
目的探讨可吸收螺钉治疗髌骨骨折的临床疗效。方法我院2010年2月17日-2013年10月28日应用日本他喜龙FIXSORB可吸收螺钉治疗髌骨骨折33例,其中男性15例,女性18例;年龄40-85岁,平均56.5岁。左侧18例,右侧15例。骨折类型:闭合性28例,开放性5例。横形骨折19例,纵形骨折14例,均为新鲜骨折。单独使用可吸收螺钉28例,联合使用张力带加强5例。结果采用国际上常用膝关节功能评价的HSS评分评定其疗效,33例患者均进行随访,随访时间为3-18个月,平均9.6个月。骨折全部愈合,未出现骨折移位、骨不连、伤口感染等并发症;末次随访时,患侧膝关节平均关节活动范围为115°±10°,HSS评分由术前的40.09分提高至89.09分。结论可吸收螺钉作为一种新型生物内固定材料,具有较好的生物相容性,可在体内溶解吸收,勿需二次手术,值得推广应用。
Objective To investigate the clinical efficacy and potential applications in treatment of patients with patellar fracture by absorbable screws. Methods Totally 33 patients with patellar fracture were treated by open reduction and fixation with FIXSORB absorbable screws from Feb. 2010 to Oct. 2013. There were 15 males and 18 females, with an average age of 56. 5 years ( ranging from 40 to 85 years old ). There were 28 closed injuries and 5 open injuries. There were 19 cases of transverse fracture and 14 cases of longitudinal fracture. A total of 28 patients were treated by single absorbable screws and 5 cases were treated by the combined use of tension band. Results The efficacy of surgery was evaluated by HSS score. All patients were achieved ideal reduction by reviewing the X-ray of the knee,and all the patients were followed up for 3 to 18 months( mean,9. 6 months). The complications of fracture displacement, nonunion, wound infection and local effusions were not observed. The HSS score was improved from pre-operatively 40. 09 to 89. 09. After 3 months of post-operation, the average range of the knee motion was 115 ° ± 10°. Conclusion As a new type of bio-fixation material, the absorbable screw has good biocompatibility, can be dissolved and absorbed in the body, doesn't need secondary operation to remove the implant, is suitable for the treatment of patellar fracture and is worthy to be widely applied in clinical practice.
