腹腔镜远端胃癌D2根治术136例临床分析

Laparoscopic distal gastrectomy with D2 lymphadenectomy for gastic cancer:a clinical analysis of 136 cases

目的 探讨腹腔镜远端胃癌D2根治术的可行性和安全性.方法 回顾性分析2011年5月～2014年5月在右江民族医学院附属医院胃肠外科进行腹腔镜远端胃癌D2根治术的136例患者的临床资料.结果 136例患者均在腹腔镜下完成远端胃癌D2根治术,在胃癌切除后均通过管形吻合器行毕Ⅱ胃十二指肠吻合重建,手术时间（207±19.7）min;出血量（121.8±39.4） ml,术后肛门排气时间（2.7±1.1）d,恢复流食时间（3.6±0.8）d.术后发生并发症3例（2.21％）,分别为切口延时愈合、腹腔内感染和短期胃瘫各1例.结论 腹腔镜远端胃癌根治术D2淋巴结清扫安全、可行.

Objective To investigate the feasibility and safety of laparoseopic distal gastreetomy with D2lymphadenectcmy. Methods Clinical records from 136 patients with distal gastric cancer undergoing laparo＂scopic distal gartrectomy hospitalized in Deparument of Gastrointestinal Surgery,The mean surgical time was （207±19.7） min. The mean blood loss was （121.8±39.4） ml. The mean time to the first postoperative flatus was （2.7±1.1） d,and the mean time to resumption of liquid diet was （3.6±0.8） d. Postoperative complication occurred in 3 patients （2.21%）, of one had prolonged incision healing, one intraperitoneal infection and one short-term gastroparesis. Conclusion Laparoscopic distal gastrectomy with D2 lymphadenectomy is safe and feasible.
