Endobutton带袢钢板治疗急性下胫腓联合损伤 被引量:5

摘要 目的探讨Endobutton带袢钢板治疗急性下胫腓联合损伤的临床疗效。方法对13例急性下胫腓联合损伤患者按外踝-后踝-内踝顺序行骨折切开复位内固定术,若伴有内侧三角韧带损伤则一并修复。对术前明确诊断下胫腓联合损伤者、术前疑似下胫腓联合损伤且术中Hook试验见腓骨外移大于4 mm或外旋应力位摄片见内侧间隙大于2 mm者均行手术固定。确认下胫腓联合复位满意后钻孔,选用袢长度合适的Endobutton纽扣钢板固定下胫腓联合。结果本组13例均获随访,平均随访时间23个月。术后患者主观评价优9例、良2例、一般1例、差1例,优良率84.6%。美国足踝骨科学会(AOFAS)评分优3例、良7例、一般2例、差1例,优良率76.9%。结论 Endobutton带袢钢板治疗急性下胫腓联合损伤有复位效果可靠、不存在内固定断裂、不需手术取出且患者可早期负重锻炼等优点,比传统采用螺钉的方法疗效更佳。
