摘要 目的探讨快速检测血浆脑利钠肽(BNP)对颅脑损伤评估的意义。方法单纯颅脑损伤41例,颅脑损伤伴颅内出血28例,入院当天行头颅CT扫描,并测定入院当天、第3天、第7天血浆BNP浓度,并与对照组30例比较。结果颅脑损伤组(单纯颅脑损伤和颅脑损伤伴颅内出血)发病当天BNP水平均高于对照组,差异有统计学意义(P<0.05),颅脑损伤伴颅内出血组发病第3天BNP逐渐升高,与入院当天比较差异有统计学意义(P<0.05),第7天升高明显且达高峰,与对照组比较差异有统计学意义(P<0.01)。单纯颅脑损伤组发病第3、7天BNP均显著下降,与入院时比较差异有统计学意义(P<0.05)。结论不同时段测定颅脑外伤患者BNP均有不同程度升高,动态观察血浆BNP含量是早期诊断和监测颅脑外伤伴颅内出血的有效指标之一。 Objective To study the clinical value of rapidly detecting BNP in estimating traumatic brain injury .Methods Sixty-nine patients with traumatic brain injury who were hospitalized in our hospital during May 2011 and May 2013 were as TBI group ,of which 41 patients with simple craniocerebral injury and 28 cases with craniocerebral injury accompanied by in-tracranial hemorrhage ,30 healthy people were as controls .Skull CT scanning was taken on the admission day ,and the plasma samples were collected to test the content of brain natriuretic peptide (BNP) on the admission day ,the 3rd day and the 7th day of treatment ,which were compared .Results The BNP content levels of TBI group was higher than that of control group on the admission day ,the difference was statistically significant (P〈0 .05) ,the level of the subgroup patients with craniocerebral accompanied by intracranial hemorrhage was higher gradually since the 3rd day of treatment(P〈0 .05) ,there was a statistically significant difference compared with that on the admission day ,and reached the peak at the 7th day(P〈0 .01) ,and there was a difference compared with control group .However ,the levels of simple craniocerebral injury subgroup decreased significantly at the 3rd day and the 7th day ,the difference was statistically significant compared with that of control group .Conclusion The BNP content level of TBI patient shows varying degrees at different time .Dynamic observation on BNP contents in plasma is a valid method for early diagnosis and surveillance on traumatic brain injury and intracranial hemorrhage .
