胃镜下注射肾上腺素联合钛夹治疗有活动出血的非静脉曲张性上消化道出血临床疗效观察

Effect of Combining Epinephrine Injection Under Gastroscope with Titanium Clip to treat Nonvaricose Upper Gastrointestinal Hemorrhage with Active Bleeding

目的:探究分析胃镜下注射肾上腺素联合钛夹治疗有活动出血的非静脉曲张性上消化道出血临床疗效。方法:选取我院自2008年5至2013年5月收治的有活动出血的非静脉曲张性上消化道出血患者80例作为临床研究对象,作为临床研究对象。将所有患者进行随机分组,每组各40例,试验组患者采用胃镜下注射肾上腺素联合钛夹治疗,对照组患者采用单纯胃镜下注射肾上腺素治疗。记录两组患者治疗后即时止血率、有效止血率及再出血率,比较两组患者不良反应情况。结果:试验组患者治疗后即时止血率、有效止血率(95.0%、90.0%)明显高于对照组(77.5%、55.0%),且其穿孔、发热、感染、周围循环衰竭等不良反应发生率(7.5%、17.5%、10.0%、2.5%)也明显较对照组偏低(27.5%、35.0%、37.5%、20.0%),P<0.05,具有统计学意义。结论:胃镜下注射肾上腺素联合钛夹治疗有活动出血的非静脉曲张性上消化道出血临床疗效显著,止血效率高,不良反应较少,具临床推广价值和意义。

Objective：To analyze clinical curative effect of combining epinephrine injection under gas-troscope with titanium clip to treat nonvaricose upper gastrointestinal hemorrhage with active bleeding .Meth-od：Chose 80 cases of nonvaricose upper gastrointestinal hemorrhage patients who have had active bleeding and have been treated at our hospital from May 2008 to May 2013 as clinical study objectives .Divided them into experimental group and control group randomly ,each group has 40 cases ,combined epinephrine injection under gastroscope with titanium clip to treat patients in experimental group , while just adopted epinephrine injection under gastroscope to treat patients in control group .Recorded the patients in two groups instant he-mostasis rate,effective hemostasis rate and rehaemorrhagia rate after treatment ,and their happening situation of adverse reaction.Result： The instant hemostasis rate and effective hemostasis rate of patients in experi-mental group were 95.0%and 90.0%,they were obviously higher than control group （ 77.5%and 55.0%） , and happening rate of tresis ,fever,infection and peripheral circulatory failure these adverse reactions in ex-perimental group were 7.5%,17.5%,10.0%and 2.5%respectively,they were obviously lower than those in control group （27.5%,35.0%,37.5%and 20.0%）,P〈0.05, There＇s statistics meaning.Conclusion：Com-bined epinephrine injection under gastroscope with titanium clip has significant clinical curative effect on the treatment of nonvaricose upper gastrointestinal hemorrhage which has active bleeding ,it has high hemostasis rate and fewer adverse reaction .It is worthy of clinical application and promotion .
