地佐辛复合丙泊酚用于无痛电子胃镜术疗效观察的meta分析

Effect of dezocine combined with propofol on painless gastroscopy: a meta analysis

目的采用meta分析的方法评价地佐辛复合丙泊酚用于无痛电子胃镜术的临床疗效及安全性。方法电子检索收集PubMed、中国知网、维普资讯网、万方数据库等建库至2013年12月发表的关于地佐辛复合丙泊酚用于无痛电子胃镜术的随机对照试验(RCT),按照Cochrane系统评价的方法评价纳入研究的质量和提取资料,采用RevMan5.2软件进行统计分析。结果纳入8个文献共计849例患者,均为中文。meta分析结果显示:与单用丙泊酚组比较,地佐辛复合组能够显著减少丙泊酚的总用量(均数差为-29.36,95%CI[-41.60,-17.12],P<0.01),缩短患者苏醒时间(均数差为-1.62,95%CI[-2.06,-1.18],P<0.01),降低术后VAS评分(均数差为-1.52,95%CI[-2.90,-0.14],P=0.03),同时减少操作刺激引起的体动反应(RR为0.35,95%CI[0.23,0.52],P<0.01),降低术中呼吸抑制(RR为0.30,95%CI[0.17,0.51],P<0.01)、血压下降等循环抑制的风险性。结论据现有研究的meta分析结果表明地佐辛复合丙泊酚用于无痛胃镜术是较安全有效的方法。

Objective To evaluate the efficacy and security of dezocine combined with propofol on painless gastroscopy by a meta-analysis.Methods Data were electronically searched in the PubMed,CNKI,VIP and Wanfang databases from the establishment of the datebases to December 2013 to identify the randomized controlled trials（RCTs） about dezocine combined with propofol on painless gastroscopy.The qualities of included RCTs were assessed according to the Cochrane system evaluation method and were statistically analyzed using the RevMan5.2 software.Results Eight studies involving 849 patients met the inclusion criteria.The meta-analysis showed that compared with propofol group,the total dosage of propofol significantly reduced in combination group（MD =-29.36,95％ CI[-41.60,-17.12],P ＜0.01）,the recovery time of patients was shortened （MD =-1.62,95％ CI[-2.06,-1.18],P ＜0.01）,postoperative VAS score reduced （MD =-1.52,95 ％ CI [-2.90,-0.14],P =0.03）,the patients＇ body response caused by operating stimulation （RR =0.35,95％ CI [0.23,0.52],P ＜ 0.01） and the incidence of respiratory depression （RR =0.30,95％ CI [0.17,0.51],P ＜ 0.01） reduced,and the dropping of blood pressure and other risk of circulation inhibition decreased.Conclusion The limited current evidences indicate that dezocine combined with propofol is effective and safe for painless gastroscopy.
