摘要
目的测定小于胎龄儿体内生长素、胰岛素样生长因子-1、瘦素水平,并分析其在生长追赶现象中的作用。方法将156例新生儿分为适于胎龄儿组与小于胎龄儿组,小于胎龄儿组根据有无生长追赶现象的出现又分为有追赶现象组和无追赶现象组。比较其发育指标及生长调节因子水平。结果小于胎龄儿组的发育指标、瘦素、胰岛素样生长因子-1均小于适于胎龄组,生长素水平显著高于适于胎龄组。适于胎龄组的体质量标准得分、身高标准得分、胰岛素样生长因子-1、瘦素水平均高于有追赶现象组与无追赶现象组,生长素水平低于有追赶现象组与无追赶现象组。生长素与体质量标准得分呈负相关,与身高标准得分无相关性;胰岛素样生长因子-1、瘦素水平与体质量标准得分、身高标准得分均呈正相关。结论小于胎龄儿在出生后存在生长素高水平,瘦素、胰岛素样生长因子-1低水平的状态,3种因子在胎儿生长发育中起着重要的调节作用。
Objective To determine Ghrelin,IGF-1 and leptin levels and analyze the role of catch-up growth phe-nomenon. Methods 156 cases of fetus arranged in small for gestational age group(SGA group)and appropriate for gestional-age group(AGA group). The SGA group was divided into CUG group and non CUG group according to the presence or absence of catch-up growth phenomenon. The growth index and growth regulating factor were compared. Results The growth index (Height,weight,head circumference),IGF-1 and leptin levels of SGA group were lower than AGA group,but Ghrelin were higher than AGA group. The Body quality standard score,height standard score,IGF-1 and leptin of AGA group were higher than CUG group and non CUG group,but the Ghrelin were lower than CUG group and non CUG group. Ghrelin was negatively related to wtSDS,and no significant correlation with HtSDS. IGF-1 and leptin were negatively related to wtSDS and HtSDS. Conclusion SGA has higher levels of Ghrelin after birth,but lower level of IGF-1 and leptin. There three factors in fetal growth play an important role in process of growth and development of fetus.
