反角涡轮钻在下颌低位水平阻生牙拔除中的改良应用初步探讨 被引量:3

摘要 目的比较传统口腔高速涡轮钻法与反角涡轮钻潜钻法配合三角切龈在相对复杂阻生下颌第三磨牙拔除中的效果。方法随机选择64例水平低位阻生下颌第三磨牙,分为实验组和对照组两组,每组32例。对照组采用传统口腔高速涡轮钻拔牙方法,实验组采用三角形切开牙龈后反角涡轮钻潜钻法拔除水平低位阻生下颌第三磨牙,记录拔牙手术时间及术后相关并发症等,并对其行统计学分析。结果实验组与对照组的平均手术时间分别为26.1分钟和36.3分钟。两组在手术时间、术后疼痛、术后肿胀、张口受限等方面,实验组均优于对照组(P<0.05)。两组在术后出血、牙龈撕裂等方面无明显差异(P>0.05)。结论反角涡轮钻潜钻法配合三角形切开牙龈是对传统口腔高速涡轮钻拔牙方法的有效改进、可缩短手术时间、减少术后疼痛、肿胀及张口受限。 Objective To compare the effect of traditional turbine drilling applications and submersible drilling conjunction with triangular gingival cutting in complex wisdom tooth extraction. Methods Sixty-four cases of lower impacted wisdom teeth were randomly divided into two groups of 32 : Experimental and control. The traditional turbine drill extraction method was used in the control group, whereas submersible drilling conjunction with triangular gingival cutting was employed in the experimental group. Surgical time and postoperative complications were recorded for statistical analysis. Results The average operative times for the experimental and control groups were 26. 1 and 36. 3 min, respectively. A significant difference in the time of surgery, postoperative pain, postoperative swelling, and trismus was found between the experimental and control groups (P 〈 0. 05 ). Results showed that the experimental group had better outcome than the control group. Conversely, no significant difference in postoperative gum bleeding and tearing was found between the two groups ( P 〉 0. 05 ). Conclusions Submersible drilling conjunction with triangular gingival cutting is an effective method of complex impacted wisdom tooth extraction because it can shorten the operation time, lessen postoperative pain and swelling, and limit mouth opening.
