2型糖尿病患者脂蛋白相关磷脂酶A2活性变化的特征 被引量:2

摘要 目的脂蛋白相关磷脂酶A2(Lipoprotein-associated Phospholipase A2,LP-PLA2)又称为血小板活化因子乙酰水化酶,是一类能够在磷脂甘油部分的Sn-2位点选择性断裂酯键的酶,在生物界中分布广泛,这类酶及其介导产物近来引起了人们的极大关注。流行病学和临床前瞻性研究均发现血浆LPPLA2质量或活性升高是心血管危险事件的独立危险因子。本研究通过检测2型糖尿病患者血浆中LPPLA2酶活性水平,探讨我国2型糖尿病患者中LP-PLA2的分布特征与影响因素。方法采用病例-对照研究,2型糖尿病230例,其中男性117例,女性113例;60周岁以上116例,不满60周岁114例;肥胖130例,非肥胖100例;无大血管合并症105例,合并大血管合并症125例。测定入组患者体重指数、血脂等常规生化指标、颈部血管超声,以及检测血浆中LP-PLA2活性,分析各分类亚组间LP-PLA2活性分布的特征。结果 LP-PLA2活性在我国2型糖尿病患者中:男性高于女性;绝经后女性高于未绝经女性;肥胖患者高于非肥胖患者;低密度脂蛋白血症组高于非低密度脂蛋白血症组;大血管合并症组高于无大血管合并症组。结论中国汉族2型糖尿病患者中,合并大血管合病变(动脉粥样硬化)的患者,血浆LpPLA2活性较高;性别、肥胖及脂代谢紊乱尤其是低密度脂蛋白血症,是影响2型糖尿病患者血浆LpPLA2活性的相关因素。 Objective Lipoprotein-associated Phospholipase A2( LP-PLA2),Also known as platelet-activating factor acetylhydrolase water activity,which is a class of phospholipid glycerol in the part of the Sn- 2fracture sites ester selective enzyme. LP- PLA2widely distributed in the biological,This kind of enzymes and mediated product recently attracted great attention. Recent epidemiological and clinical prospective studies have found that the quality of plasma,or LP- PLA2activity increased the risk of cardiovascular events is an independent risk factor. This research through the detection of diabetes plasma lipoprotein related phospholipid A2enzyme activity level in type 2 diabetes patients lipoprotein related phospholipid A2 distribution and influencing factors.Materials Using case- control study,type 2 diabetes 230 cases,including male cases 117 cases,women 113cases,Over 60 years old were 116 cases,under 60 years old were 114 cases,Obesity,obesity 130 cases,non-obesity 100 cases,No vascular complications 105 cases,combined with vascular complications,the determination of 125 patients body- mass index,the conventional biochemical indicators,blood vessels,and ultrasonic testing neck. LP- PLA2activity,analyzing the classification and groups of lipoprotein related phospholipid A2active distribution characteristics. Results Lipoprotein related phospholipid in A2activity in patients with type 2 diabetes in men than women; Post- menopausal women than not postmenopausal women,Obese patients than the obese patients,Low density lipoprotein blood group than the low density lipoprotein hematic disease,Vascular complications than without vascular complications group. Conclusion Chinese of the Han nationality patients with type2 diabetes,All combined macrovascular disease( atherosclerosis) of the patients,plasma Lp- PLA2activity in higher. Sex,obesity and lipid metabolism disorders,especially low- density lipoprotein levels,that affect patients with type 2 diabetes plasma Lp- PLA2 activity in the relevant factors.
