妊娠足月剖宫产仰卧位低血压综合征22例分析

The Analysis of 22 Cases of Full- time of Pregnancy and Cesarean Section Supine Hypotension Syndrome

目的探讨妊娠足月剖宫产仰卧位综合症的发病及防治。方法 2012年10月至2013年10月妊娠足月行剖宫产结束分娩的354例产妇,其中出现仰卧位低血压综合征22例,分析孕妇年龄、麻醉方式、剖宫产平卧时间、体位对足月孕妇发生仰卧位低血压综合征的影响。结果妊娠足月行剖宫产结束分娩的354例产妇,发生仰卧位低血压综合征的有22例,初产妇13例(占59.1%),经产妇(包括二次剖宫产患者)9例(占40.9%)。接受腰硬联合麻醉的是3例(占13.6%),接受硬膜外麻醉的是19例(占86.4%)。5例发生在麻醉前(占22.7%),17例发生于麻醉后待麻醉奏效期间(占77.3%)。结论妊娠足月剖宫产期间仰卧时间长、平卧位、硬膜外麻醉易引起妊娠足月剖宫产仰卧位低血压综合征。

Objective To explore the full-term pregnancy and cesarean supine syndrome’s incidence and prevention. Method From 2012 October to 2013 October,in the 354 cases of who were in the end of full- time pregnancy and cesarean section deliveries,supine hypotension syndrome occurring 22 cases; and we analyze the aspects of the maternal age,anesthesia,cesarean supine time and postural on full- term pregnant women with supine hypotension syndrome. Results In the 354 cases of maternal with full- term pregnancy and maternal cesarean section who were in the end of delivery,22 cases pregnant women suffer from supine hypotension syndrome; primiparas were 13 cases（ 59. 1%） and maternal（ including secondary cesarean patients） were 9 cases（ 40.9%）. Three cases（ 13.6%） were received lumbar epidural anesthesia and 19（ 86. 4%） cases were received epidural anesthesia. Five cases occurred before anesthesia（ 22. 7%） and 17 cases occurred after anesthesia（ 77. 3%）. Conclusions During the full- time pregnancy and cesarean,long term of supine time,supine position and epidural anesthesia can lead to full- time pregnancy and cesarean supine hypotension syndrome.
