肺心病心电图演变规律探讨

Approach of the EKG development rules of cor pulmonale

目的 :探讨肺心病的心电图演变规律 ,以减少肺心病的漏诊。方法 :①对 76例单纯肺心病缓解期和复发期记录心电图 ,对其阳性率进行自身对照 ;② 6 1例肺心病合并冠心病在肺心病发作期的心电图阳性率与上述 76例单纯肺心病 (对照组 )复发时的阳性率进行对照。结果 :①单纯肺心病缓解期心电图阳性率 6 3.2 % ,低于复发期的 92 .1% ,两者差异有非常显著意义 (P <0 .0 1) ;②肺心病合并冠心病后心电图阳性率 78.7% ,与对照组 92 .1%比较 ,差异也有显著意义 (P <0 .0 5 )。结论 :①单纯肺心病缓解期心电图阳性率低于发作期 ;②一些肺心病人并存冠心病后的EKG演变依然遵循上述规律 ;③另一些则由于并存冠心病 ,肺心病心电图阳性率下降。

Objective: To decrease some cases of cor pulmonale failed to be diagnosed through the reseach the EKG development rules of cor pulmonale.Methods: ①We compared respectively the EKG positive rate of the 76 patients sufferring from simple cor pulmonale at their exacerbation stage and lysis stage by self-control study.②Also,we compared the EKG positive rate of 61 cases illing with cor pulmonale complicated by coronary heart disease (CHD) at their cor pulmonale exacerbation stage with that of above 76 cases at their relapse(The control group).Results:① The EKG positive rate (63.2%)when simple cor pulmonale relieved was significantly lower than that(92.1%),when the disease relapsed (P<0.01).②when complicated by CHD,the EKG positive rate of cor pulmonale(78.7%)was also significantly lower than that of the control group (92.1%,P<0.05). Conclusions: ①The EKG positive rate when simple cor pulmonale has relieved is lower than that when it attacks.②The EKG development of some cor pulmonale keeps above rule when complicating by CHD.③ It is CHD that reduces the EKG positive rate of the other cor pulmonale when associated with CHD.
