泪道逆行置管术与泪囊鼻腔吻合术治疗慢性泪囊炎观察

Clinical effects on retrograde intubation of silicone lacrimal drainage tube compared with dacryocystorhinostomy for chronic dacryocystitis

目的 对比近年开展的逆行泪道置管手术（A组）与泪囊鼻腔吻合术（B组）两种手术方法治疗慢性泪囊炎的疗效.方法 临床病例对照研究.对2012年1月至2013年1月在长海医院眼科就诊的慢性泪囊炎患者据门诊电脑就诊序号随机分组,A组46例（46只眼）行新型泪道置管术;B组44例（44只眼）行泪囊鼻腔吻合术,并统计分析两组疗效、手术与住院时间、手术费用的差异.结果 A组46只眼,其中女性38只眼;B组44只眼,其中女性36只眼.A组总体有效率86.96％ （40/46）,略低于B组95.45％ （42/44）.但A组平均手术时间、住院时间与B组比较,均明显优于B组,两组差异有统计学意义（P＜0.01）.结论 逆行泪道置管手术快速便捷,值得推广,手术技巧的提高和器械的改进有望进一步提高有效率.

Objective To compare the effect of retrograde intubation of silicone lacrimal drainage （group A） and dacryocystorhinostomy （group B） in treating chronic dacryocystitis.Methods Chronic dacryocystitis patients at Changhai hospital outpatient from January 2012 to January 2013 were assigned randomly according to outpatient number in computer.Group A （46 eyes） underwent retrograde intubation of silicone lacrimal drainage,group B （44 eyes） underwent dacryocystorhinostomy.Effective rate,operative time,hospital stay and surgery costs were statistically analyzed between the two groups.Results Group A （46 eyes） had effective rate of 86.96% （40/46）,and Group B （44 eyes） 95.45% （42/ 44）.Group A had significantly shorter operative time,hospital stay than the group B （P 〈0.01）.Condusions Retrograde intubation of silicone lacrimal drainage is quick and easy performed,improved surgical techniques and equipment may improve clinical efficiency in treating chronic dacryocystitis.
