头孢他啶联合左氧氟沙星治疗心力衰竭患者肺部感染的疗效分析

Effect analysis of ceftazidime combined with levofloxacin for cardiac failure patients with pulmonary infection

目的探讨头孢他啶联合左氧氟沙星治疗心力衰竭患者肺部感染的临床疗效,为临床资料提供参考依据。方法收集2009年1月-2013年6月124例心力衰竭发生肺部感染的患者临床资料,将其随机分为对照组和观察组,各62例;两组患者均给予抗心律失常、控制钠盐摄入等常规综合治疗,对照组给予左氧氟沙星,治疗组左氧氟沙星联合头孢他啶;观察两组的疗效、炎症因子的变化及细菌的清除率,数据采用SPSS17.0软件进行分析。结果治疗组总有效率为91.9%,显著高于对照组的79.7%;治疗后两组血脑钠肽(BNP)、白介素-6(IL-6)及TNF-α均较治疗前明显下降,且治疗组BNP显著低于对照组(P<0.05);治疗组的细菌总清除率为69.4%,显著高于对照组的49.0%,两组比较,差异均有统计学意义(P<0.05)。结论左氧氟沙星与头孢他啶治疗心力衰竭合并肺部感染,可迅速控制感染症状,促进病原菌清除,改善心功能。

OBJECTIVE To explore clinical effect of ceftazidime combined with levofloxacin for cardiac failure with pulmonary infection. METHODS A total of 124 cases of cardiac failure and pulmonary infection patients were randomly divided into two groups. All the cases were given regular treatment including bed rest, antiarrhythmic, and control of sodium intake. The control group was treated with levofloxacin, and the treatment group was treated with ceftazidime combined with levofloxacin. The clinical effect, changes in inflammatory factors, clearance rate of bacteria in 2 groups before and after treatment were observed and compared. RESULTS The total effective rate in the treatment group was 91.9%, higher than 79.7% in the control group. After treatment, the level of BNP, IL- 6 and TNF-α in both groups were all decreased, and the treatment group had more remarkable decrease in BNP than the control group （P〈0.05）. The total effective rate in the treatment group was 69.4%, which was significantly higher than that in the control group 49.0% （P〈0.05）. CONCLUSION Combination use of ceftazidime and levofloxaein for cardiac failure with pulmonary infection is safe and effective, which is helpful to control infection rapidly, promote pathogen clearance, and improve heart function.
