Role of Toll-like receptor 4 in mitigation of inflammatory responses following myocardial ischemia-reperfusion injury by sufentanil post-conditioning in rats

摘要 目的 评价Toll样受体4(TLR4)在舒芬太尼后处理减轻大鼠心肌缺血再灌注损伤后炎性反应中的作用.方法 清洁级健康成年雄性Wistar大鼠30只,体重250~300 g,采用随机数字表法分为3组(n=10):假手术组(S组);缺血再灌注组(IR组)采用结扎大鼠心脏左冠状动脉前降支30min恢复灌注的方法制备心肌缺血再灌注损伤模型;舒芬太尼后处理组(SP组)缺血30 min,于再灌注前5 min静脉注射舒芬太尼0.6μg/kg.于再灌注120 min时取腹主动脉血及心肌标本,采用ELISA法测定血清IL-6和TNF-α浓度及心肌TLR4含量.结果 与S组比较,其余2组血清IL-6和TNF-α浓度及心肌TLR4含量升高(P<0.05);与IR组比较,SP组血清IL-6和TNF-α浓度及心肌TLR4含量降低(P<0.05).结论 舒芬太尼后处理可减轻大鼠心肌缺血再灌注损伤后炎性反应,其机制可能与下调心肌组织TLR4表达有关. Objective To evaluate the role of Toll-like receptor 4 (TLR4) in mitigation of inflammatory responses following myocardial ischemia-reperfusion (I/R) injury by sufentanil postconditioning in rats.Methods Thirty Wistar rats were randomly divided into 3 groups (n =10 each):sham operation group (S group); I/R group; sufentanil postconditioning group (SP group).Myocardial I/R was produced by temporary ligation of left anterior descending branch of coronary artery for 30 min followed by 120 min reperfusion.In group SP,sufentanil 0.6 μg/kg was injected intravenously at 5 min before reperfusion.The myocardial specimens and blood samples were taken at the end of 120 min reperfusion for determination of the interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) concentrations and TLR4 content (by ELISA).Results Compared with S group,the levels of TLR4,IL-6 and TNF-α were significantly increased in the other two groups.Comparedwith I/R group,the levels of TLR4,IL-6 and TNF-α were significantly decreased in group SP.Conclusion The mechanism by which sufentanil postconditioning mitigates inflammatory responses following myocardial I/R injury may be related to downregulation of TLR4 expression in myocardial tissues of rats.
