胸腔内注射卡提素治疗恶性胸腔积液的临床观察

Efficacy of Pleural Injection with Extro Bacillus Calmetlegurin for Malignant Pleural Effusion

目的探讨胸腔内注射卡提素治疗胸腔积液的近期疗效和毒副反应。方法 45例经病理学确诊的恶性胸腔积液患者随机分为常规治疗组(22例,丝裂霉素、顺铂胸腔内注射);卡提素组(23例,在常规治疗基础上加用卡提素),比较2组患者胸腔积液变化情况。结果 45例恶性胸腔积液患者的总有效率86.7%。卡提素组近期疗效优于常规治疗组(P<0.05)。2组毒副反应相近(P>0.05)。治疗后,卡提素组生活质量优于常规治疗组(P<0.05)。结论在丝裂霉素、顺铂胸腔内注射等常规治疗的基础上,加用卡提素治疗恶性胸腔积液疗效优于常规治疗。

Objective To observe the efficacy and toxicities of pleural injection with extro bacillus calmetlegurin for malignant pleural effusion. Methods Forty-five patients with malignant pleural effusion were randomly divided into two groups,22 patients of the routine treatment group received the pleural injection with mitomycin plus cisplatin,and 23 patients of the extro bacillus calmetlegurin group received the pleural injection with mitomycin plus cisplatin combined with extro bacillus calmetlegurin. Results Of the 45 patients,the response rate was 86. 7%. The short-term efficacy of the extro bacillus calmetlegurin group was better than that of the routine treatment group（ P 〈0. 05）. There was no statistical difference in the toxicities between the two groups（ P 〉0. 05）. After the treatment,the quality of life of the extro bacillus calmetlegurin group was better than that of the routine treatment group（ P 〈0. 05）. Conclusion For the patients with malignant pleural effusion,the efficacy of pleural injection with extro bacillus calmetlegurin combined with mitomycin plus cisplatin is better than that of pleural injection with mitomycin plus cisplatin.
