摘要 目的探讨人类表皮生长因子受体2(Her2)在介导乳腺癌细胞上皮间质转换中的作用。方法采用免疫组化方法检测156例乳腺癌组织标本中Her2、上皮样细胞标志E-钙黏素(E-cadherin)、间质样细胞标志N-钙黏素(N-cadherin)表达,根据Her2表达情况将患者分为Her2阳性组与Her2阴性组,比较两组患者E-cadherin N-cadherin表达情况。结果与Her2阴性组比较,Her2阳性组E-cadherin阳性表达率明显降低(75%VS39.47%),N-cadherin明显升高(30%VS61.84%),两组比较差异均具有统计学意义(P<0.05);E-cadherin、N-cadherin两者表达与TNM分期、淋巴结转移有关(P<0.05),表现为TNM分期和淋巴结转移程度越严重,E-cadherin阳性表达越低,N-cadherin阳性表达越高;Her2、N-cadherin阳性表达患者的病死率更高,而E-cadherin阴性表达患者的病死率更高,阴性组与阳性组比较差异具有统计学意义(P<0.05)。结论Her2阳性表达促进乳腺癌细胞上皮间质转换过程,同时增强乳腺癌的浸润转移能力,严重影响患者的预后情况。 Objective To discuss the effect of human epidermal growth factor receptor 2 (Her2) mediating breast cancer cells epithelial mesenchymal transition. Methods 156 cases Her2, E-cadherin N-cadherin express in breast cancer tissues were detected by using immunohisto-chemical. According to the level of Her2 expression, patients were divided into Her2 positive group and Her2 negative group. Results Compared with Her2 negative group, E-cadherin positive expression rates of Her2 positive group decreased significantly(75%VS39.47%), and N-cadherin positive expression rates increased significantly(30%VS61.84%) (P〈0.05). E-cadherin, N-cadherin expression are closely related to TNM staging and the degree of lymph node metastasis (P〈0.05). E-cadherin expression is lower and N-cadherin expression is higher with the aggravation of TNM staging and lymph node metastasis. The fatality rate of Her2, N-cadherin positive expression patients is higher, and E-cadherin negative expression patients is higher(P〈0.05). Conclusion Her2 positive expression can promote EMT process of breast cancer cells and enhance the invasion and metastasis degree of breast cancer. The expression of Her2 could influence the prognosis of patients.
