The monitoring points of vancomycin use in intensive care units （ICU） are an important issue for the clinical pharmacists. We summarized working points of vancomycin use in ICU by analyzing typical cases. Clinical pharmacists could monitor vancomycin use by designing regiments, analyzing the fluctuation of its plasma concentrations, and recongnizing ADR of vancomycin. Effective communication between pharmacists and nurses could insure plasma samples accuracy. The participation of clinical pharmacists in clinical treatment can promote the rational use of vancomycin and reduce the ADR. Pharmacists play an important role in monitoring vancomycin use in ICU.
