心力衰竭并心房颤动再同步化治疗1例 被引量:3

摘要 1 病例资料 患者为男性,77岁,因“反复胸闷气促5年,再发2个月”入院.既往糖尿病5年.5年前劳累后胸闷、气促,休息数分钟缓解.3个月前夜间突发胸闷,伴大汗并晕厥,诊断急性心肌梗死行急诊经皮冠状动脉介入(PCI)治疗,在左前降支植入2枚支架.近1个月夜间不能平卧,双下肢水肿.查体:血压100/70 mm Hg(1 mm Hg=0.133 kPa),神志清,颈静脉怒张,双肺呼吸音减低,心率62次/min,律不齐,第一心音强弱不等,脉搏短促.腹平软,双下肢水肿.实验室检查提示肌酐(Cr) 171 μmol/L,氨基末端脑钠肽前体(NT-proBNP) 21 659 pg/mL.其余指标大致正常.辅助检查:心电图示心房颤动伴右束支传导阻滞(图1).超声心动图显示左心房内径45 mm,右心室内径16 mm,左心室舒张末内径66 mm,射血分数内径42%.
