老年抑郁症中医证候类型与体质相关性调查研究

Correlation research for traditional Chinese medicine syndrome type and constitution of senile depression

目的通过临床调查研究,探讨老年抑郁症与中医体质的关系。方法对符合纳入标准的566名老年抑郁症患者的证型情况进行归纳、评分,并按《中医体质分类与判定》标准判定这些患者的体质类型,分析老年抑郁症患者最常见的体质类型;用判别分析、因子分析等方法对数据进行处理,总结出老年抑郁症证候规律;用线性回归分析老年抑郁症患者证候类型与体质的相关性,并通过频数分析总结出各体质类型常见的证候要素和复合证型。结果老年患者体质类型的出现率分别是气虚质、阴虚质、阳虚质;对老年抑郁症证型分析结果显示,肝肾阴虚证、肝郁脾虚证是老年抑郁症的常见证候。结论老年抑郁症与患者体质有密切关系,气虚质、阴虚质是老年抑郁症患者的重要致病特质。

Objective To investigate the relationship between the senile depression and constitution of traditional Chinese medicine. Methods 566 elderly patients with depression syndromes who meet the inclusion criteria were summarized and graded in according to the “ TCM constitution classification and judgment” standard physical type in these patients were determined, the most common physical type of elderly patients with depression was identi-fied; geriatric depression syndromes rules were summed with methods discriminant analysis, factor analysis; the correlation between elderly patients with depression syndromes and physique was analyzed by linear regression and the various physical types of common certificate were analyzed and summarized by frequency designate elements and complex syndromes. Results The frequency of occurrence for physical type of elderly patients was qi deficiency, yin deficiency and yang quality. Geriatric depression syndromes analysis showed that syndrome of yin deficiency of liver and kidney and stagnation of liver qi and spleen deficiency is a common syndrome of depression in elderly. Concluzion There is a close relationship between in elderly patients with depression and constitution. Qi deficiency and Yin deficiency is an important pathogenic characteristics of elderly patients with depression.
