摘要 目的观察乌司他丁在改善非小细胞肺癌患者放疗后肺功能中的作用。方法 120名非小细胞肺癌患者随机双盲分入实验组和对照组,实验组在放疗开始前3天及放疗中前7天接受乌司他丁治疗,同时对照组患者使用安慰剂治疗。观察放疗后放射性肺损伤发生率及程度。患者在放疗开始前,放疗后第8、24周时行肺功能评估。结果放疗后实验组患者放射性损伤发生率及程度以及FEV1,DLCO和FVC等指标下降程度明显低于对照组,第8周时实验组FVC值明显低于对照组,在24周时FEV1,DLCO和FVC下降程度明显低于对照组。结论乌司他丁能有效降低非小细胞肺癌患者放疗后放射性肺损伤发生率并减轻肺功能下降程度。 Objective To investigate the effect of ulinastatin on pulmonary function of patients with NSCLC after radiation therapy. Methods 120 patients with NSCLC were randomly divided into two double-blind groups. The treatment group was treated with ulinastatin for 3 days prior to and for the first 7 days of radiotherapy, and the control group was treated with placebo for 10 days. RILI and pulmonary function were evaluated at 8 and 24 weeks af-ter radiation therapy. Results No adverse toxicological effect of treatment with ulinastatin was observed. The inci-dence and grade of RILI were significantly lower in the treatment group than in the control group (P〈0. 05). There was significant difference in FVC at the 8th week between the two groups (P〈0. 05). Reductions in the values of FEV1 , DLCO and FVC from baseline and at the 24th week were significantly smaller in the treatment group than those in the control group (P〈0. 05). Conclusion Ulinastatin can significantly reduce the incidence and grade of RILI and improve PF in the treatment of NSCLC patients after radiotherapy.
