牛痘疫苗致炎兔皮提取物颈部硬膜外注射治疗颈椎间盘突出症的临床研究

The clinic research on treatment of cervical disc herniation with cervical epidural lateral interspace extract from rabbit skin inflamed by vaccinia virus injection

目的探讨牛痘疫苗致炎兔皮提取物颈部硬膜外注射治疗颈椎间盘突出症的临床效果。方法将90例行颈硬膜外阻滞治疗的颈椎间盘突出症患者随机分成3组,每组各30例。A组:牛痘疫苗致炎兔皮提取物+复方倍他米松+罗哌卡因,B组:牛痘疫苗致炎兔皮提取物+罗哌卡因,C组:复方倍他米松+罗哌卡因+维生素B。记录治疗第5天的VAS、症状体征评分及副作用;用Macnab标准评估治疗后1、6和12个月的疗效。结果注药第5天3组VAS、症状体征等评分分别为:A组显著优于B组,B组显著优于C组(均P<0.05);3组副作用无显著差异(P>0.05);治疗后1、6和12个月3组的Macnab有效率分别为A组显著优于B组,B组显著优于C组(均P<0.05)。结论硬膜外穿刺注射牛痘疫苗致炎兔皮提取物+复方倍他米松+罗哌卡因比局麻药加激素更能提高颈椎病患者的近期及远期疗效。

Objective To explore the clinical effect of cervical epidural lateral interspace extract from rabbit skin Inflamed by vaccinia virus injection on cervical disc herniation.Methods 90 patients with cervical disc herniation treated by cervical epidural block were randomly divided into group A,group B and group C.Group A was treated with extract from rabbit skin Inflamed by vaccinia virus,ropivacaine and compound betamethasone.Group B was treated with extract from rabbit skin Inflamed by vaccinia virus and ropivacaine.Group C was treated with compound betamethasone,ropivacaine and vitamin B.The VAS score,symptoms and signs and side effect were recorded.The curative effect was evaluated by Macnab criteria 1 month,6 months and 12 months after treatment.Results The score of VAS and symptoms and signs in the group A was better than that of group B （P〈0.05）,and those of group B was better than that of group C （P 〈0.05）.The differences of side effect in the three groups were no significant（P〉0.05）.Clinical efficacy rate in group A was better than that of group B（P〈0.05）,and those of group B was better than that of group C （P〈0.05） 1 month,6 months and 12 months after treatment.Conclusion Cervical disc herniation treatment with extract from rabbit skin Inflamed by vaccinia virus,ropivacaine and compound betamethasone injection can more effectively improve the recent and long-term therapeutic effect cure rate than local anesthetics and hormone.
