压力疗法在内眦赘皮矫正术后瘢痕增生挛缩防治中的疗效观察

Efficiency of pressure therapy on prevention and treatment of scar proliferation and contracture after epicanthus operation

目的观察压力疗法在内眦赘皮矫正术后瘢痕增生挛缩防治中的疗效。方法筛选2010年3月至2013年3月期间我科内眦赘皮女性患者240例,年龄（28.4±1.2）岁,均行＂Z＂字成形内眦赘皮矫正术。其中120例为防治组,年龄（26.2±0.8）岁,术后1月开始采用压力疗法,嘱患者用手指将内眦处隆起包块压向内侧鼻骨处,按摩压力大小为10-15 mmHg。防治组中再分为每天1次组40例,3次组40例,6次组40例;另外120例为对照组,年龄（29.1±0.6）岁,无任何防治。观察、对比2组瘢痕增生挛缩率。结果经过术后1年的随访,防治组采用压力疗法后术区瘢痕变平,无痛痒症状,触之柔软,瘢痕无充血,内眦角度保持术毕效果,瘢痕增生挛缩率9.2%;对照组瘢痕增生挛缩率37.5%,统计学分析差别具有非常显著性（P〈0.01）;防治组中每天3次组效果最好（P〈0.05）。结论每天3次压力疗法在内眦赘皮矫正术后瘢痕增生挛缩防治中的疗效显著。

Objective To determine the effect of pressure therapy on the prevention and treatment of scar proliferation and contracture after epicanthus operation.Methods From March 2010 to March 2013,240 epicanthus patients（average age 28.4 ± 1.2） in our department were treated by＂Z＂plastic epicanthus operation.After operation,120 patients（average age 26.2 ± 0.8） were chosen for pressure therapy in 1 month after operation（treatment group）.These patients were asked to press the epicanthus scar to the nasal bone nearby with fingers and the pressure was 10- 15 mmHg and then divided into 3 groups：40 patients for 1 time per day,40 patients for 3 times per day and 40 patients for 6 times per day.The other 120 patients（average age 29.1 ±0.6） did not receive such pressure therapy（control group）.Results After 1 year follow-up in the pressure therapy group,the scar was flat and soft with no pain and itch.The epicanthus angel kept the same appearance.The scar proliferation and contracture ratio was 9.2%,significantly different from that of the control group 37.5%（P 0.01）.In the treatment group,the subgroup with 3 times of pressure therapy per day had the best effect（P 0.05）.Conclusion The pressure therapy with 3 times per day has significant effects on the prevention and treatment of scar proliferation and contracture after epicanthus operation.
