异常体动脉供血左肺下叶与左下肺隔离症的CT表现对比分析

The Comparative Analysis on CT Findings between Anomalous Systemic Artery to Left Lower Lobe and Pulmonary Sequestration in Left Lower Lobe

目的对比分析异常体动脉供血左肺下叶(ASALLL)与左下肺隔离症(PS)的CT表现。方法回顾性分析16例确诊为ASALLL及25例确诊为左下肺PS的CT表现,并对比分析两者的病灶形态、与支气管关系、供血动脉起源及直径等。结果反复咳嗽及咳痰分别见于21例及15例左下肺PS患者,咯血见于16例ASALLL患者(P<0.05)。左肺下叶呈毛玻璃影见于16例异ASALLL患者,左肺下叶肿块见于22例左下肺PS患者(P<0.01)。16例ASALLL患者的左肺下叶支气管正常,25例左下肺PS与支气管不相连(P<0.01)。ASALLL的异常动脉直径(11.88±1.13mm)大于左下肺PS的异常动脉直径(5.96±0.98mm)(P<0.01)。结论ASALLL与左下肺PS的CT表现不同,CT增强扫描可确诊ASALLL。

Objective The aim of this study was to comparatively study computed tomography （CT） findings of anomalous systemic artery to left lower lobe （ASALLL） and pulmonary sequestration （PS） in left lower lobe （LLL）. Methods The CT findings of 16 patients with ASALLL and 25 patients with PS in left lower lobe were retrospectively analyzed. Results There was statistically significant difference in hemoptysis, having been seen in 16 （100%） patients with ASALLL, 6 （24%） of 25 patients with PS （P〈0.05）. Ground glass opacity, normal bronchial connection with LLL, dilated left inferior pulmonary veins, and absence of the interlobar artery distal to the origin of the superior segmental artery in LLL were found in 16 （100%） patients with ASALLL, while none was seen in PS （P〈0.01）. Mass was less frequent in ASALLL （0%） than in PS （88%） （P〈0.01）. The mean diameter of anomalous artery （11.88 ±1.13mm） in ASALLL was significantly larger than that （5.96 ±0.98mm） in PS （P〈0.01）. Lesion in LLL and anomalous artery arising from thoracic aorta were seen sinfilarly in SASLLL （100%） and PS （72%）. Conclusion The CT findings of ASALLL and PS in left lower lobe were different. ASALLL could be diagnosed by enhanced CT scan.
