摘要 目的探讨基于磁共振PROSET序列的三点计划扫描(3PPS)技术在盆区及臀区坐骨神经单一层面显示中的初步应用。方法采用Philips Achieva1.5T MRI扫描仪。首先,对20名健康志愿者及6例腰腿痛患者的坐骨神经行冠状位PROSET序列扫描。然后,应用3PPS技术,分别在不同平面的冠状位图像上选取三点,扫描单侧坐骨神经。结果应用3PPS技术,全部26条坐骨神经中24条均可在单一层面全长显示其盆区及臀区,扫描时间约为7分钟。2例不能在单一层面全长显示。正常坐骨神经在PROSET序列上呈条纹状稍高信号,走形自然、连续,神经周围的血管呈高信号,肌肉组织呈较高信号,神经与其邻近组织解剖关系清晰。6例腰腿痛患者中显示邻近血管压迫2例,血管与神经关系密切,脂肪间隙消失;神经源性肿瘤1例,能够清晰观察到肿瘤位于坐骨神经近端。结论基于PROSET序列的3PPS技术能够在单一层面清晰地显示盆区及臀区坐骨神经全长。 Objective To explore PROSET MR Sequence -based 3PPS's initial application in displaying the unitary layer of sciatic nerve in pelvic and pygal fields. Methods With Philips Achieva 1.ST MRI, PROSET sequence scanning for the coronal part of sciatic nerve is firstly performed for 20 healthy subjects and 6 subjects with pain in the waist and legs. Then, 3PPS was applied to scan unilateral sciatic nerve by choosing 3 points in different plans of the coronal image. Results With 3PPS, after seven-minute scanning, 24 out of the total 26 unitary layer of sciatic nerves can be displayed in full length. Normal sciatic nerves display linear marking and slightly higher signal with natural and uninterrupted sequence. Besides, veins encircling nerves expressed high signal, musculatures displayed comparatively high signal and the distinction was shown between nerves and its surrounding tissues. Among the 6 subjects with pare in waist and legs, there were two with vascular compression in adjacent vessels and disappearance of lipoid space; there was one with neurogenic tmnor observable clearly from the proxial end of sciatic nerves. Conclusion PILOSET MR Sequence-based 3PPS displays with clarity the unitary layer of sciatic nerves in pelvic and pygal fields in fnll length.
