摘要 目的探讨彩色多普勒超声对乳腺恶性肿瘤的诊断价值。方法选取经手术后病理证实的乳腺癌患者74例,分析其二维超声图像特征、血流分布特点、血流参数收缩期血流峰值(PSV)及阻力指数(RI)。结果超声诊断乳腺癌66例,误诊8例,诊断符合率为89.2%(66/74)。74例乳腺癌中,边界不清、边缘呈毛刺/蟹足、周边强回声晕、沙砾样钙化、纵横比≥1、后方回声衰减及淋巴结肿大的检出率分别为74.8%、64.9%、39.2%、48.6%、28.4%、47.3%及36.5%。血流分级0-Ⅰ级者21例,Ⅱ-Ⅲ级者53例;其中58例RI为0.62-0.82,PSV为9.32-21.38 cm/s。结论彩色多普勒超声对乳腺癌的早期发现和诊断有重要价值。 Objective To explore the value of color Doppler ultrasound in the diagnosis of malignant breast tumor. Methods A total of 74 cases of pathologically confirmed breast cancers were included.Data of two-dimensional ultrasound sonographic features,blood characteristics of the distribution,peak systolic velocity(PSV) and resistance index(RI) ofhemodynamic parameters were analyzed.Results Sixty-six cases were diagnosed correctly and six cases were misdiagnosed, the accuracy for the breast cancer diagnosis was 89.2% (66/74).The occurrence rate of unclear boundaries, edges burr/crab feet, surrounding halo, micro-calcification, vertical/horizontal ratio ≥1, the posterior attenuation, enlarged lymph nodes was 74.8% , 64.9%, 39.2%, 48.6%, 28.4%,47.3% and 36.5%, respectively.In all 74 cases, 21 cases color signal scale were 0-Ⅰ , the other 53 cases were Ⅱ-Ⅲ .The RI was 0.62-0.82,the PSV was 9.32%21.38 cm/s in 58 cases.Conclusion Color Doppler ultrasound plays an important role for early breast cancer detection and diagnosis.
