中药穴位贴敷联合顺铂胸腔灌注治疗恶性胸腔积液随机平行对照研究

A Parallelism Randomized Controlled Clinical Trial on Chinese Medicine Acupoint Sticking Combined Cisplatin Injected Into Thoracic Cavity in the Treatment of Malignant Pleural Effusion

［目的］观察中药穴位贴敷联合顺铂胸腔灌注治疗恶性胸腔积液疗效。［方法］使用随机平行对照方法,将44例住院患者按就诊顺序编号随机分为两组。对照组20例顺铂,60mg/次,1次/周,胸腔内注药。治疗组24例中药穴位贴敷（甘遂去心、大戟去皮、白芥子各400g,按1 ：1 ：1比例,粉碎研细,过100目筛后备用）,粉剂25g＋50%生姜汁调匀成膏状,外敷双阴陵泉、双三阴交、患侧大包穴,4 ～ 6h/次,2次/周;顺铂胸腔灌注同对照组。连续治疗3周为1疗程。观测临床症状、胸水、不良反应。治疗1疗程,判定疗效。［结果］治疗组CR9例,PR11例,NC2例,PD2例,总有效率91.67%。对照组CR7例,PR8例,NC3例,PD2例,总有效率90.00%。临床疗效两组间无明显差异（P＞0.05）。腔积液深度两组均有改善（P＜0.05,P＜0.01）,治疗组改善优于对照组（P＜0.05,P＜0.01）。［结论］中药穴位贴敷联合顺铂胸腔灌注治疗恶性胸腔积液,疗效满意,值得推广。

[ Objective ] To observe the traditional Chinese medicine acupoint sticking combined cisplatin injected into thoracic cavity in the treatment of malignant pleural effusion. [ Methods ]With the randomized, controlled method, 44 eases of hospitalized patients in clinic order Numbers randomly divided into two groups. Control group 20 eases of eisplatin, 60 mg/time, 1 time per week, thoracic cavity injection. Treatment group 24 eases of traditional Chinese medicine acupuncture point attach （ euphorbia kansui, spurge 400 g to 400 g heart to skin, white mustard seed 400 g, according to the proportion： , crush porphyrization, reserve through 100 mesh sieve ）, powder 25g ＋ 50% raw ginger juice to make a paste, apply double Yin ling spring, two three vaginal intercourse, side bag hole, 4 - 6 h/time, 2 times per week;eisplatin treatment with the control group. Continuous treatment 3 weeks to 1 course of treatment. Observation of clinical symptoms, chest, chest water depth, water colour to exceed adverse reactions. After a course of treatment efficacy. [ Result ] the treatment group CR9, PR11, NC2 eases, PD2 eases, the total effective rate was 91.67%. Control group shape and PR8, NC3 eases, PD2 cases, the total effective rate was 90.00%. Clinical curative effect between the two groups have no significant difference （ P 〉 0.05 ） . Depth of pleural effusion in the two groups before treatment were improved （ P〈 0.01, P 〈 0.01 ）, in the treatment of 4 weeks, there was no obvious difference between the two groups （P 〉 0.05 ）, but in the treatment of 2 weeks and 3 weeks, depth of pleural effusion in treatment group was obviously less than control group （P 〈 0.01, P 〈 0.05 ） . [ Conclusion ] Aeupoint sticking combined eisplatin injected into thoracic cavity in the treatment of malignant pleural effusion, curative effect is satisfied, is worth promoting.
