胃肠间质瘤住院病人营养风险筛查

Screening of nutritional risk in hospital patients with gastrointestinal stromal tumors

目的:筛查胃肠间质瘤(GIST)住院病人的营养风险发生情况。方法:对在我院胃肠外科住院的235例GIST病人进行营养风险筛查,按照肿瘤危险性将其分为极低危险性组、低危险性组、中危险性组和高危险性组;按肿瘤发生部位分胃、十二指肠、小肠、结肠、直肠、系膜和腹腔其他部位等调查,并比较各组营养风险发生率。结果:235例病人均完成NRS 2002筛查,营养不足和营养风险的发生率分别为55例(23.40%)和80例(34.04%),其中极低危险性组病人营养风险发生率为28.57%、低危险性组为22.41%、中危险性组为25%、高危险性组为50%。组间比较发现,高危险性组病人营养不良风险性明显高于其他组(P<0.05);按照肿瘤部位分析病人营养风险发生率,胃间质瘤为28.15%、十二指肠42.85%、小肠35.89%、结肠18.75%、直肠37.5%、系膜75%、腹腔其他部位71.42%。系膜和腹腔其他部位肿瘤病人营养不良风险性明显高于其他组(P<0.05)。结论:GIST住院病人存在一定的营养风险,高危险性和系膜、腹腔其他部位肿瘤病人较其他GIST病人营养风险发生率更高。

Objective：Assessment of nutritional risk status was made among hospitalized patients with gastrointestinal stromal tumor（GIST）.Methods：The nutritional risk features of 235 GIST cases were collected.All GIST cases were classified into very low,low,intermediate and high-risk groups respectively according to the histological risk classification.Nuritional risk was also evaluated and compared according to tumor location.Results：All patient＇s assessments of nutritional risk screening were completed.The incidences of undernutrition and nutritional risk were 23.40％ （55 cases） and 34.04％ （80 cases）.The nutritional risk incidence in very low,low,intermediate and high-risk GIST groups were 28.57％,22.41％,25％ and 50％,respectively.The high-risk group had significantly higher proportion of nutritional risk than the other three groups （P ＜ 0.05）.According to the tumor location,the incidences of nutritional risk for stomach,duodenum,small intestine,colon,rectum,mesenteric and other parts of abdominal cavity were 28.15％,42.85％,35.89％,18.75％,37.5％,75％ and 71.42％,respectively.The incidence of nutritional risk amongst patients in the last two groups was significantly lower than that in the other groups （P ＜ 0.05）.Conclusion：There is convincing evidence that nutritional risk is prevalent among GIST patients.GIST patients with high-risk and in mesenteric and other parts of abdominal cavity have the highest incidences of nutritional risk.
