血液透析联合前列地尔治疗急性肾功能衰竭的疗效观察

目的 探讨血液透析联合前列地尔治疗急性肾功能衰竭的临床疗效。方法 选取合肥市第一人民医院自2008年9月~2013年12月收治的90例急性肾功能衰竭患者,随机分为观察组和对照组（n=45）。对照组行血液透析治疗,观察组行血液透析联合前列地尔治疗,对2组患者治疗前后肌酐（Cr）、尿素（UR）、内生肌酐清除率（CCR）、尿量、尿素氮（BUN）等指标及急性生理学及慢性健康状况评分系统（APACHEⅡ）评分进行对比。结果 2组患者治疗前APACHEⅡ评分为观察组（32.4±6.5）分,对照组（31.3±5.7）分;治疗后为观察组（15.5±4.1）分,对照组（20.4±4.9）分,经对比可知2组患者治疗前APACHEⅡ评分比较差异无统计学意义,治疗后观察组明显优于对照组（P〈0.05）。2组患者治疗前Cr、UR、CCR、尿量、BUN等各项指标水平之间均无显著差异,之后2组各指标均有显著改善,且观察组改善情况显著优于对照组（P〈0.05）。结论 给予急性肾功能衰竭患者血液透析联合前列地尔治疗可有效地改善患者各项指标,临床效果显著。

Objective To observe the clinical efifcacy of hemodialysis combined with alprostadil treating acute renal failure.Methods 90 cases with acute renal failure treated in our hospital from September 2008 to December 2013 were selected and divided into two groups randomly, the observation group and the control group, each group for 45 cases. The control group was treated with hemodialysis, and the observation group was treated with hemodialysis combined with alprostadil. Cr,UR, CCR, urine volume, BUN and APACHEⅡscore of two groups were compared.Results The two groups of patients before treatment APACHE score for the observation group （32.4 ± 6.5）, control group （31.3 ± 5.7）; the observation group after treatment （15.5 ± 4.1）, control group （20.4 ± 4.9）, compared with two groups of patients before treatment APACHE score had no signiifcant difference, after treatment, the observation group was better than the control group （P〈0.05）. Between UR, CCR, Cr, urine volume, BUN and other indexes were no signiifcant difference between the level of treatment of two groups of patients, after the two groups were signiifcantly improved, and the observation group improved more signiifcantly than the control group （P〈0.05）.Conclusion Hemodialysis combined with alprostadil treating acute renal failure can evidently improve indicators of patients, and has evident clinical effect.
