可达龙联合琥珀酸美托洛尔缓释片对植入埋藏式自动心脏复律除颤器的致死性室性心律失常患者的疗效观察

Efect of Cordarone jointing Metoprolol succinate tablets in patients with lethal ventricular arrhythmias who have implanted automatic implantable cardioverter defibrillator

目的:评估可达龙联合琥珀酸美托洛尔缓释片对植入埋藏式自动心脏复律除颤器(ICD)治疗致死性室性心律失常的疗效。方法:选取我院2006-01-2012-07室性致死性心律失常患者43例,行ICD置入术,术后随机分为对照组(可达龙)20例与治疗组(可达龙联合琥珀酸美托洛尔缓释片)23例,随访6个月,比较两者临床事件发生率、起搏器程控参数和超声心动图指标等。结果:所有患者随访期间未出现晕厥及猝死。治疗组临床事件、心律失常事件发生率低于对照组,而且LAD、LVEDD、E/A得以改善。两组患者心室电极导线感知、起搏阈值、阻抗、除颤阈值与用药前比较无变化。结论:可达龙联合琥珀酸美托洛尔缓释片治疗因致死性室性心律失常植入ICD患者的疗效优于单用可达龙。

Objective：To obsever the effect and salty of automatic implantable cardioverter defibrillator combi- nation with Cordarone and Metoprolol succinate tablets on lethal ventricular arrhythmias. Method： All 43 patients with fatal ventricular arrhythmias, who had implanted automatic implantable cardioverter defibrillator in our hospi- tal from January 2006 to July 2012, were randomly divided into control group （n=20） and treatment group （n= 23）. Control group was treated with Cordarone, and treatment group was treated with Cordarone jointing Meto- prolol succinate tablets. Two groups were followed for 6 months to compared the incidence of clinical events, the pacemaker programmed parameters and color Doppler index. Result~ All patients had no syncope or sudden death during follow-up. Compared with control group, treatment groupr s incidence of clinical events and arrhythmic e- vents decreased morely, and LAD,I.VEDD and E/A improved morely. Two groups of patients had no significant change about ventricular electrode perception, threshold, impedance and defibrillation thresholds before or after treatment. Conclusion：It is effective and safe for automatic implantable cardioverter defibrillator combination with Cordarone and Metoprolol succinate tablets on lethal ventricular arrhythmias.
