MANIFESTATION OF SCHMORL'S NODES ON MRI AND ITS ASSOCIATION WITH LUMBAR DEGENERATION

摘要 目的探讨腰椎Schmorl结节的检出率、影像学表现及其与腰椎退变的关系。方法行腰椎MRI检查的病人218例,对Schmorl结节在不同性别之间的检出率、检出部位、终板形态及信号、椎间盘信号、椎体骨赘等指标进行对比分析。结果在218例病人中发现Schmorl结节者82例(182个结节),男45例,女37例;182个Schmorl结节中位于终板前1/3者26个,中1/3者67个,后1/3者89个;123个结节所在终板边缘椎体信号为ModicⅠ型,21个为ModicⅡ型,38个为ModicⅢ型;1 727个凹陷终板共有66个Schmorl结节,105个弓形终板共有7个Schmorl结节,322个直形终板共有104个Schmorl结节,26个断裂及不规则终板共有5个Schmorl结节;946个伴椎体前缘骨赘的终板发现114个Schmorl结节,1 234个无骨赘终板发现68个Schmorl结节;与Schmorl结节相关的椎间盘有142个,评分0分的有29个,1分的有83个,2分的有16个,3分的有14个。结论Schmorl结节于男性多见,并且与其邻近的终板及椎间盘退变明显相关。 Objective To explore the detection rate and MRI manifestations of Schmorl's nodes and their association with lumbar degeneration. Methods A MRI examination was carried out in 218 patients. The detection rate of Schmorl's nodes in different genders, site of detection, shape of endplate and its signal, intervetebral signal and vertebral osteophyte were compared and analyzed. Results Of the 218 patients, 182 Schmorl' s nodes were found in 82 patients-male 45, female 37. Of the 182 modes,26 lacated anterior 1/3 of endplate,67 at middle 1/3 and 89 at posterior 1/3. Of the 182 nodes, 123 were Modic I , 21 were Modic Ⅱ, and 38 were Modic Ⅲ. There were 66 Schmorl's nodes found in 1 727 pouch endplates, seven nodes in 105 arch-form endplates, 104 nodes in 322 straight endplates, five nodes in 26 ruptured and irregular endplates. And 114 nodes were found in 946 endplates with anterior vertebral osteophyte,68 nodes were found in 1 234 endplate without anterior vertebral osteophyte. There were 142 discs that associated with Schmorl's nodes, 29 discs rating 0 of the score,83 rating 1, 16 rating 2 and 14 rating 3. Conclusion Schmorl's nodes mostly seen in male and are closely correlated with adjacent endplates and intervertebral disc degeneration.
